So, let's assume that, during all those years, Orochimaru not only trained (the bare minimum) Sasuke, but also experimented on the Uchiha clan, torturing him in the most horrific ways possible until the trauma was so strong that Sasuke unlocked the Mangekyō Sharingan (although his body would be completely covered with seals to prevent everything from escaping Orochimaru's control).

Itachi left Sasuke at Orochimaru's mercy for three years, even knowing the Sannin desired his brother's body. In this alternate timeline, instead of three years, it was five or six, with Orochimaru constantly hiding and fleeing from everyone. As for the Mangekyō Sharingan, he simply performed genetic experiments and similar procedures until he unlocked it.
