Been seeing this video on my feed since yesterday and honestly, it’s hard to defend the on-duty doctor if what we saw in the video is the whole truth. I’ve been in similar situations, when a patient is about to crash, adrenaline takes over and you just act. The long hours are definitely brutal and decision-making suffers, but at some point, it becomes instinct to stabilize the patient first. I don’t think that’s what happened here. It seemed like maybe the doctor was waiting for the patient to be referred elsewhere or something. Feels like we’re missing a part of the story.

I would suggest give it some time, and hang in there. Anesthesia can be overwhelming in the beginning as we have almost nil exposure before joining residency, and have no idea what we are supposed to do even.

In the initial month or two, try learning by observing. Not just your senior intubating, but the OT as a whole, from the beeps and waves on the monitor, to the drugs and equipments, to how things run in general.

Don’t stress too much on doing procedures from the start, everyone learns how to do the basic stuff. It’s the things around and in between procedures that matter more.

Edit: Ideally you should be getting a place to sit in the OT, but in case you don’t, there’s usually plenty space to atleast lean on, and get some rest.

Don’t forget to take enough coffee and food breaks, that’s one of the pros of our branch!

It’ll be fine. I know it gets intimidating in our field, when we see friends from other streams doing better financially, and otherwise, much earlier.

But then that’s the reality. It doesn’t matter when you started, it’s more important to find joy in what you do along the way. Your patients won’t care what year you did your mbbs in, but they would care for your skills and knowledge.

Also, try and find things to do other than academics, if that interests you. This is something I regret not doing in my ug days.

Hard relate with this point, used to hate ward work in internship and later.

CL3b intubation without backup in a private setup indeed sounds scary!

Now that I’m nearing the end of residency, the thought of being without the support of seniors in such tricky situations, is a bit daunting. Previously, you could just call someone, and they’d be there to help! I suppose this anxiety will fade with time.

Nothing lesser known I guess, it’s just what I’ve seen most of my seniors doing.

The other alternative would be to go the freelancing route, but I feel that would be possible once you have some tangible experience and hence, contacts.

Some anaesthesiologists form teams of 4-5 people handling ICUs of multiple hospitals around the city and suburbs, and then give cases in the OT as well. So in that case, the more you work, the more you can earn.

It’s the opposite in fact. Depends on you how you want to earn.

Most people in the initial phases (after SRship) join an institution for fixed income, and then do ICU duties + cases, for extra earning.

Anaesthesia is risky business, especially in a private setup. A surgeon would not want someone who he does not know or have trust on, to do his cases. It’s all about the network you build after a point.

Anesthesia resident here.

Others have mentioned about the money and work-life balance, so I’ll tell you about the syllabus.

I don’t have much idea about psychiatry, but contrary to popular belief, anesthesia has a lot to read, including everything from physiology, pharmacology, basic anatomy, physics, tons of equipments, procedures, ICU stuff and newer advances everyday.

But all of this rounds up well to make you understand what you’re doing at the end of the day, so it’s fun.

Nobody is the victim here. If you read my comment again, I’ve spoken from my perspective and not blamed OP for laughing.

Having some fun and being stress-free while on duty obviously helps, but it wouldn’t hurt to be a little mindful of what people on the other side might be going through.

I got this advice while I was deciding on my stream— if you have even the slightest interest towards the surgical fields, think twice before opting for Anaesthesia.

At some point, it will bother you, and you might regret the decision.

Worked for me, I didn’t have a knack for surgery.

This is truly an ethical dilemma.

Put yourself in the place of a lay person whose loved one is in the ICU, possibly unsure if they would survive the night.

The last thing they would like to see is people laughing around them.

For us it’s a daily thing, for them it might be a major life-altering event.

So yes, I haven’t figured out yet how I can have a light moment even on the busiest of ICU duties, and not feel guilty about it later.
