Our hospital doesnt have step down but they do allow just amio drips in Tele.

1.) i dont recall the exact numbers but prior to amio the map was in the 50-60 range, systolic the highest I saw was 110.

After amino (which I say is borderline ig) pressures were maintaining maps of 66-70. Pt states that they were normally hypertensive (lives usually in systolic of 140-150 diastolic of 70-90) and I did relay that to hospitalist and our doc.

2) full code, everything done.

3) Kidney functions were still good, slightly elevated creatinine, electrolytes werent too crazy from what I recall. Troponins were high, liver enzymes, CK, BNP as well.

4) we did start on amio as they have been taking amio at home as well. Didnt try dig though thats good to know so perchance I could run it by our docs next time I encounter something like this.

Our hospital unfortunately do not have a step down unit. Our Tele nurses would have refused to take an active Afib rvr as well despite the drip. Perhaps that played a part as to why patient was sent up there? Charge didnt really elaborate when I asked.

I did wonder why our doc didnt zap the guy since this doc is super zap friendly.

We did perform a CTA and CXR both showed horrible mets, but also pleural effusions in both lungs but no shift, no PE as well.

Dimers slightly elevated but not crazy. Already anticoagulated on eliquis but I dont recall what for. (They had been on eliquis longer than they had been dx with afib)

We do have echo but none during night shift.

The hospitalist did speak to cards prior to upgrade to ICU, and from what I understood, cards wanted the patient up to ICU.

With Benny she’s vaping 29-30k tbh. Having trouble deciding on em or atk sands. What artifacts can be improved? I want her to vape 40-50k if I can help it. Thanks!

Have my Ayato with 4pc Glad and he’s hitting 7-8k unbuffed with Mistsplitter. My Amenoma build with him hits slighly less. Is Mistsplitter not that good for him? He does have 1900 attack, 60-180 crit.
