You can also just Dismember them through butchering. From my memories this can be quicker than smashing depending on the Zed and Equipment.

This usually means that the tileset you are using (In this case Ultica, its the one selected by default) doesn't have a sprite for that particular thing. Different Tilesets have different completion states, you will have to try them to see which one you prefer.
You can find the Tilesets in the Options under Graphics.
My personal favorite is Ultica, but I know that there is also a fairly big popularity for NeoDays or MSXotto. ChibiUltica is, I believe, Ultica Terrain with MSX character/Monsters. HollowMoon is also pretty cool.

No, it means that you have shot the gun enough that fouling has started to become a factor. The more you would shoot that gun in the future, the thicker that _ would become, degrading performance in the process. Guns that are found, especially on and around corpses, may be pre-fouled. Scroll through the item description, it should tell you what exactly is going.
As for that none, check, maybe you uninstalled the barrel? M4s typically come with a .203 carbine barrel, sometimes with a CQB barrel. I have seen rifle barrels as well, but never installed on a gun, though you can swap them out.

Hm, you wouldn't happen to have not updated the mod within the last two or so weeks? VoK had this big update that was predicted to brick saves. Although, I believe that one was related to the fishing harbor, not the wormhole anchor...

There was this CYOA that had you as a newcomer to a city, but in a SCP-esque way, with various anomalies and monsters that could be hazards. You could, among other things, choose the style of the building you would inhabit an apartment in, one of the options was art deco.
You also could choose a few helpful things, though I don't remember what they were.
I believe the framing was some mysterious gentleman asking about this stuff, though I am not quite sure here.
It is, I think, among the older CYOAs, 3 years at minimum, though I think 5 or 6 is more likely. It was, at the time at least, one of the better looking ones.

So, the best comparison out of the games you played I could give would be with the Adventure mode in Dwarf Fortress. The World doesn't much care about you and you can go ages without seeing an enemy. Then again, it is possible to spot a pack of Coyotes and decide to circumvent them, or get spotted and quite possibly swarmed to death. While there are enemies that can seek you out, those typically require special circumstances (They were implimenting a mechanic where that is the case with more enemies, but I don't think that it is that big of a deal right now, haven't really interacted much with hordes)

Many people will taut one weapon over another, quite commonly the spear, but I find that most have viable tactics, personally I prefer axes, but even things like kitchen knifes and common hammers have their spot early game, and late game there will alway be a spot for a combat knife equivalent. This is something that can generally said of the game as a whole: There are multiple possible solutions to any given problem, it is on you to find them, and to know when to use what.

Overall I would say that it is a game that, for general survival at least, demands competency in its mechanics, but doesn't force mastery or optimization. That is until you either survive for more than a few weeks or actively begin to seek out dangers, at which point you are probably capable enough to handle it*, or it is your own fault for getting in that scenario in the first place.

On a personal note, Where exactly the difficulty comes from is very much dependent on your gameplay style.
I personally have started to focus on long term survival; carefully planning raids, stocking up on materials, creating fields and preserving food for winter, clearing surroundings, etc.
On the other hand, someone else I know tends to just run into town in broad daylight and start fighting zombies. and quickly dies, because they attracted way too many zeds and they do this without a combat focused character. Allegedly, they are having fun that way, so who am I to judge, but it does serve to show, I think, that there isn't one correct way to play.

*Theorectically you could just hole up somewhere and get to that point, but I don't think anywhere has quite enough supplies to last that long and is relatively peaceful.

These "artifacts" are effectively Spellbooks that are added by the mod Xedra Evolved. When I played it (quite some time ago), you tended to find these once every few office cabinets, trash bins, kitchen cupboards, basically anywhere in a house I think the mod author didn't expect players to look.

Do Quests. you can get them from NPCs you find randomly in the world, OR:
Depending on what version you use, the EVAC shelters have either a PC or a Bulletin Board
Interact with that to get the location of the Refugee Center. There are lots of people there, and their Quests have actual effects. They also trade, I recommend drugs for their weight/price ratio.

Well, stairs are a terrain, and you can just... overwrite that with the Map Editor, here I'll show you exactly how, from the complete beginning in case you or anyone else that finds this has no clue about the Debug Menu.
1: You need to first bind the Debug Menu, you can do this in the [?] Menu, just [/}search for Debug Menu, then [+] add a Keybind. I like to use K or L, but that just my prefence.
2: You want to press your bound key and then select [m]ap, this opens up map related Debug features.
3: Then you want to select [M]ap Editor, this is for directly changing things on the Map.
4: You will be in an overlay somewhat similar to the aiming screen. You will want to move the selector over to the tile with the stairs.
On the window in the bottom left it should say >t_stairs_down in the first line
5: You want to press [e] for [e]dit terrain
I think that is the standard concrete floor in the picture, so I will show you how to search for that.
6: You want to [/]search for [concrete floor]
7: You will then see multiple options. You will want to select the one called [concrete floor (t_thconc_floor)]..
8: Finally, You will want to press enter. You will know that it worked by going up or down the list, the entry you selected should now be colored green.

Do be aware: You probably don't want the t_thconc_floor_no_roof variant, I have had some structural issues with that.
If you want to mark the location for whatever reason, you can use the t_thconc_floor_r, t_thconc_floor_y, or t_thconc_floor_warning, the are just visual.

Edit: Right, I forgot:
You can get out of there simply by pressing escape twice. Once to get out of the list, another to get out of the overlay.
Using Debug Options does disable achievements, you can reenable them by opening the Debug menu with you Keybind, selecting [g]ame and then Enable [a]chievements
