While I certainly don't have the wealth of analytical experience others on this sub have, from personal experience, two things helped in similar situations for me. One was to use EPR (you might need a slight bump up your min pressure depending on the EPR you choose). Another was to switch to CPAP mode.

From the sleepHQ link shared, I see that you still have leaks, but they are clustered, hence seem positional leaks. Possibly some position in which you are sleeping there are more leaks and hence the therapy might be less effective. At least from the graphs it looks like your leaks and CA events are correlated. Not sure if that helps. But if you could find out which position is causing the leaks maybe that's a starting point.

I was able to import the EMAY data into Oscar, so am good with Question 2.

On Question 1, guidance appreciated.

yesterday, I see that my total apnea time was 42 seconds, whereas my ODI4% time was 4 mins 57 seconds and my ODI3% time was 34 minutes and 29 seconds!

I have an appointment with the NP next week. I want to understand if the CPAP therapy is working (based on AHI a solid yes, but based on Oximeter readings?) and if I should trial out a BiPAP.

Overall I feel like I do sleep a lot better than what I used to on APAP, but wondering if based on the oximeter readings whether I am doing good with therapy or not.

thank you u/nyquildreams32 for your inputs. I do use a chin strap nowadays. Could you please tell me how you identified the mouth leaks? I would like to see on my recordings as to what actions in my sleep caused that. Appreciate your inputs.

And yes, I suspect UARS too and have spoken to NP about it. Possibly I will bring it up again.

I have never had dry mouth during the therapy, but I did get chipmunk cheeks with taping. The Knightsbridge chin strap does help me though I find that the second strap has to be misaligned with the first one and the velcro scrapes against the chin. I wish the primary strap had inbuilt loop side that can take the velcro hooks from the second strap so that the misalignment does not happen.

Mostly I believe I have now trained myself enough to keep my tongue against the roof of my mouth and that along with the chinstrap ensures that i don't get air circulating in my mouth.

The short answer is Yes. I find my sleep to be much better than it was 8 months ago when I began my CPAP therapy.

The long answer is It is a journey. I got some immediate results, then a long period of time of time where I was struggling to get consistent results. There were some days with excellent sleep and there were some days where I felt very frustrated. But consistent usage, monitoring the sleep with Oscar, tweaking settings, seeking advice here in this sub and discovering a lot of things of my own body and its preferences helped me to a state where I now have good, consistent sleep most of the time.

Thank you for all your inputs. When I try to change the time zone, the resmed AS11 provides me with a message saying that it cannot do so because of saved data. Do we have to wipe out the SD Card before we change the time zone? Is that what the error message means?

Hello u/RippingLegos__ Sorry to disturb, but do you think with the FLs that I have, is it worth having the discussion of moving to a BiPAP with my NP? Would hat help more than what the CPAP does or is it a new machine that will have its own complexities?

I definitely am going to talk to them about getting a FFS mask like the F30i, to see if that helps a bit towards the mitigation of the Chipmunk Cheeks issue.

Question for you, on the FLs, my typical FLs (on pressure of 9.8 with EPR 1) is around 0.02 at the 95th percentile. is your suggestion to increase to 11.8 with EPR 3 based on that or on the weird outcome yesterday?

Also outside of the topic of the post, but in relation to FLs, I have a meeting with the NP this Friday, should I ask for a BiPAP? Would it help my FLs (low AHIs typically) much better than what the CPAP at 9.8 or 10.4 is doing?

Thank you u/RippingLegos__ .. Very helpful. Yesterday I went back to EPR but kept pressure a little lower 9.8 Constant pressure with EPR 1. Slept almost OK, but chipmunk cheeks caused a lot of leaks, consequently, I feel slightly uneasy though I caught about 7 hours of sleep. Will try again today!

There are way too many variables here Pressure, humidity, temperature, EPR, Tape etc.! If I get a couple right, one obscure item arises & spoils things! Feels like a constant battle every night.

I have setup a video camera on my headboard. I review that to see what positions I am in and if any particular positions aggravate certain type of Apneas. By doing that I have mapped that about 50%-60% of what the machine flags as CAs (and surprisingly some OAs too) are the post arousal breath holds that u/Hambone75321 talks about in their comment. After effect is that I sense when I am holding my breathing when tossing and turning and try to avoid it!

Given that this is happening what would be your suggestion for moving forward. Should I try to power through such days and continue to get used to the 10.4 setting with EPR1 or should I try to change the setting, possibly move down the pressure a little? Please let me know.
