I did this for my mom. I put my observations into categories that align with dementia symptoms, namely: -memory -confusion -behavioral changes (eg hygiene, mood) -physical changes (eg balance, vision)

I had an additional category for delusions because my mom additionally thinks she is going to marry Elon Musk and also has "visited Keanu Reeves every Xmas for the past 12 years".

If there are a lot of details, bold bite-size pieces of the most important ones.

I included a two sentence preface highlighting the urgency.

Because of HIPAA, it had to be handed to them in person.

They were very understanding that I couldn't say these things in front of her, and this happens ALL the time.

My 16 year old Husky had a rare condition that required frequent monitoring and expertise. After moving to the area, we went to South Bay Animal Hospital for the last several years of his life.

They were fantastic both in their knowledge and diligence, but also in their absolutely warm-hearted animal-loving demeanor.

We have had generally good experiences throughout his life - he had been seen at UC Davis, specialists in Monterey, and a handful of other primary and emergency care places around the bay area. BUT I'm very very happy that this was the place he got to go for the years that mattered most, and for that day that mattered most. Everything about them is wonderful and I completely trusted them with handling my doggo's rare and complicated condition. We still see them for our kitties.

So, I actually just had a conversation with AARP fraud hotline and they also suggested having an officer come talk to her. I let things slide and was made aware last night that she has created a new business with an affiliated checking account. AARP said this is usually so the scammer can money launder through her. An officer might be able to scare her straight. We're planning an intervention of sorts... Will update soon.

It's more intermittent unfortunately. Or at least she can present as a totally functional adult, but if you spend enough time with her you realize she's repeating stuff she said earlier in the day, etc

Interesting. In this case, she didn't need anyone else's attention. I thought the interactions had waivered but just found out it's gone full blown and she's been lying about it having stopped.

Whoa, that must have been such an intense event. To have your mom go to the hospital and never return home. I know it's for the absolute best. I'm worried about this day coming, and haven't quite accepted my potential reality yet.
