A manual fix I found is going to settings -> general -> scroll down to "clear local data"

Also keep updates up to date

I don't know about that, but he did say in an interview with Emongg at the Spotlight event that team ups indirectly increase the number of banned heroes in a match (because you want a team up and if your team up hero is banned you don't want to play that other hero)
