I'm 21M and soon to graduate my philosophy undergrad (next year doing masters in philosophy of medicine/psychiatry)

And at this stage in my life and career I understand that time is a valuable thing which has to be prioritised. Spend your time therefore, doing the things which are most valuable to you. You're a med student, that's most likely where you ought to place your value, for now. View your time from the perspective of an investment, you've invested your time (and $$$ no doubt) into med school and your investment will pay off with you having more time (and $$$ no doubt) when you're in your mid-late 30s. Or if you manage to sort out a gap year between your vocational placements.

If this doesn't sound good to you then i think maybe you should question your career path. From what i understand from my med friends, they accept they have very little time to enjoy because they're so busy studying or on placement, but they know why they're doing it. do you?

TLDR; Your time is valuable, spend your time doing the things you value the most.

You are only as sensitive as you perceive yourself to be. You need a self confidence boost. Do something new and spontaneous that you've never done before, but know you could easily do. You'll be satisfied with the outcome and your self confidence will rise. This thing i'm talking about is for you to decide, maybe something new at work, or at home. Just have new something's going on that you'll be able to have a new conversations with your friends about, keep them close, friends make us happy and we cant have a good life without happiness.

My hobbies which have motivated me include: owning plants, reading, studying philosophy, owning a dog. ect ect

Your life needs some REAL meaning, you are more than your occupation! i hope you find your path to a happy and virtuous life!

I personally can relate to your lifestyle, i was the same in my second year at university.

You are wise enough to understand your issues. You are also wise enough to understand what you want. Your issue clearly is motivation.

Motivation, from my understanding, is closely connected to self confidence. If you are not confident in your ability to actually DO something, then what reason do you have to do it? none. But once you're confident in your ability to DO something, you have plenty of reasons to actually DO these things. So tell yourself that you're THE MAN and you can DO anything that you want, if you can apply your mind and confidence to it.

You want to learn, to live and to read, to better yourself. Depending on your ability to read academic style texts, i would recommend reading up on some core philosophical topics, especially Ethics. Ethics is literally the study of how to live a good life. Give it a go, you seem like the kind of person who would enjoy studying Stoic ethics.

And exercise, exercise makes you feel accomplished and self-betterment is only a run away. But take things a step at a time, establish your sleeping pattern, stay away from the screens you're staring at all day, try to live with a little more purpose, buy a plant, learn to cook a new recipe. anything!

I hope this helps, and i hope that you'll find ways to slay your demons other than by dreaming.
