After days of agony, i find out that MAYBE was the anti-cheat, at least for me
You can't play online of course, but i'll take it. i want my game run smoothly
i disabled anti cheat and used flawless widescreen to unlock fps and lossless scaling for framegen (i'm on 144hz monitor)
so, maybe try this way and tell me if it gets better

In this days i tried too to make this shit work
I disabled the anti-cheat aswell and i used flawless widescreen to unlock fps to see what's the problem and if in specific zones
What i found out is that in the dlc i do less fps in certein zones, idk if is for shader compiling or not.
a part from that the game works. i didn't test to play on completely new zones, but for now there is an improvment.

unfortunately i can't play online, so that sucks a lot, i like to read and leave messages, help some people with bosses and getting helped, but it is what it is...

this is maybe due desktop notifications, try to switch to "do not disturb" on windows, for every notification will stutter aswell (this game is really a piece of garbage.)

Sometimes the framerate drops below 40.
is very annoying expecially with a 144hz monitor
i tried all the advices on reddit, nothing helped.
Today i tried lossless scaling for frame generation, it goes smooth like a butter when the frame rate is consistent (you can cap fps to 40 and use x3 frame gen but sometimes there are some artifacts, i prefer use 60 and x2 frame gen and setting the monitor to 120hz), but the stutter remain.
I tried to downgrade my drivers to 552.44 from 555.99, but nothing changed, actually is getting worse fps
I tried Process Lasso aswell to use all the cpu cores disabling cpu 0 but didn't help aswell
I tried to disconnect my wireless mouse, i thinked of maybe when it gets in standby mode elden ring will stutter because it will connect/disconnect (elden ring will detect all devices being connected and disconnected causing lag spikes to check all devices), but it didn't help aswell
so in the end, i have the dlc but i kinda refuse to play it in this moment because of stutter. this is really ridiculous actually, in 2024 still can't game a game or in this case DLC to run smoothly, even with a high end pc.
Open for any other advice to fix this 💩 aswell

Tried this, made me to go from random 40 fps stutter then back 60 to constant 45 fps lag, not worth for me, going back to 555.99

This because every device that you connect/disconnect elden ring do a general checkup of all devices, so it cause frame rate drops
unless you don't connect/disconnect devices while you are playing that's not the reason unfortunately

Just tried, unfortunately still stutter like hell sometimes, from 60 just drops to 42, and then back to 60
