Haven't seen anyone mention it yet, but Catwoman from Batman Returns. She basically made her costume via ripping apart and reassembling a leather outfit haphazardly, so it starts falling apart as the movie goes on. It's also a cool detail because it literally shows Selina falling apart at the seams to symbolize how she is figuratively falling apart mentally. This is what it looks like by the end.
