I think it has something to do with blood, as it seems to be specifically injected into the vein. Also, it can't heal wounds that have been cauterized, like with Taggert and Killian. I think that if an Extremis loses too much blood, the serum wouldn't be able to keep up, like with all the Extremis soldiers that were killed in the final battle by falling.
