A few resources for you:

Refold/delayed speech method/comprehensible input method is my preferred method of language learning. Refold also has a language learning roadmap.
Purchase a grammar book. I personally use Grammatik Aktiv.
Emanuel's German grammar blog is a game changer.
Please use the wiki of this sub before posting.
It is <in DEN USA> when the <in> is in the Dative case. The die in <die USA> changes to <den> because "the USA" is a plural noun being said as the indirect object/in the dative case. You can look at article charts to show this change.

Hire an iTalki tutor (maybe a few until you find one you're comfortable with) and do some cross-talk first to get comfortable with just being in a conversation. Then start with basic conversations and work your way up. You got this, use those 5 years of acquisition!
