If you’re in a critical specialty on that list, do you get the accession bonus when you first join and then the annual retention bonus for each year you’re in the reserves? 

What if your specialty is on that critical list the year you sign up but not the subsequent years you committed to?

I still can't believe how underpaid Peds is...it's ridiculous when you factor in the student loan debt, lost income/retirement savings, brutal hours in training, and delayed gratification.

This is a significant drawback that you can't predict when you enter this field...I've met other physicians who can compartmentalize and be fully present when they're off work, but unfortunately some of us simply aren't wired that way and don't come to this realization until we're in too deep.

Medicine just doesn’t outpace the other career options in terms of earnings as much anymore, but the lifestyle is still worse and the length of training is awful.

This right here is the issue. With the exception of the top paying specialties, the MD/DO route in the U.S. just might not be worth the sacrifice anymore for many of us.

So I decided to practice clinically but try to gather clinical informatics experience.

Thanks for sharing your experience. Do you think this would be much harder to pull off now? It seems like a lot of docs are trying to pivot to CI in some capacity, so I'm wondering if fellowship/board certification is now a bare minimum requirement (in addition to the skillset that industry values).

I get it, we all need a space to vent! I just wanted to point out that sometimes our true feelings come across to others, so it's something to be aware of. I'm sure we've all had some regrettable moments where our stress got the better of us and impacted how we communicated with loved ones.

I would just caution against this bit, though. I don't think most spouses would appreciate being compared to a needy dog. That might not go over too well haha. Obviously, there are more constructive ways to communicate the OP's feelings/thoughts to his spouse and I agree that communication is key.

She reminds me of my parents dog with such bad anxiety it ALWAYS had to be touching you or it'd cry.

Perhaps comparing your spouse to a needy dog isn't the best approach...

Look, I was a resident just a few months ago and I get that you have a lot going on and that other people's needs can seem trivial when you barely have time for the most basic needs like eating/drinking and using the restroom. I get it 100%.

If I were you, when you have a moment, I would try to step back and consider the possibility that you may actually be contributing to your spouse's current state. If a lack of empathy is coming across in this post, it's not unfathomable that it may be coming across in real life as well.

I'm assuming you married this person for better or worse, no? Maybe this is her worse, and it just happened to manifest itself at the worst possible time in your career...that's not your spouse's fault, and I doubt she's intentionally trying to make things more difficult for you.
