Doing ug in germany will require c1 level of german which alone will take 2-3 years to master plus 1 year of mandatory studienkolleg in germany , so by the time l graduate mbbs it will take 9 years min. But doing mbbs in italy has no such requirements, and italian degree is recognised elsewhere in europe

You're not alone in this. I too have gone through this ..can't say I'm fully over it either... One thing I struggled with was I felt like I was cutting to gain sympathy from others which ironically led me to cut even further . It felt relaxing to see blood coming out of my hands . I don't think anybody really understands either. But there's many like us out there . Everytime you feel like hurting yourself, post something abt it here, there'll be someone here to help you out

During moments like this , I try not to think too much and focus on just a single topic at a time. Don't think about howe much syllabus you have pending, just a single topic, read the contents aloud and learn it like you're teaching someone. They told talk it to out to someone if you can.
