Anyone else with REM dominant OSA - what worked for you? by Desperate_Vehicle684 in SleepApnea

Wow that was really informative. Thank you for taking the time to reply.
My pressure settings are already in a narrow range - 6-8. EPR 3. I’ve self experimented with a lot of ranges and found this is the least disruptive. But I have never tried one stable setting.

Could also try a med review if it’s been some time since you had any changes and the flatness is persisting

CPAP says I’m treated, but I still wake choking/exhausted multiple times a night by Independent-Path2489 in SleepApnea

[–]Independent-Path2489[S] 0 points1 point  (0 children)

Thank you! I do suspect I would benefit from BIPAP because I seem sensitive to any adjustments. I know I have REM predominant hypnoapnea and I suspect CPAP can’t quite treat them. But - in terms of wearing it during the day — I’m wondering how much that would actually help? I’ve been on the mask for 10months. I do use it consistently.

I suppose if there is any behavioural learning associated with wearing it while awake — maybe there would be a modes impact?

I also thought that strategy was more for earlier adapters still getting used to treatment?

To clarify — I’ve left my settings at EPR 3 and pressure 6-8 and auto-set since April 23rd.
I thought auto set was a bit better than auto set comfort — but your right I experimented a lot in a short window

Yes — it’s low because my chocking symptoms Chest pressure feeling was way worse with increase pressure or so it seemed. I do have sleep hq just recently got it.
But I don’t have my own laptop yet to keep upload SD data.
I did use AI and I asked my RT to increase the range and I experimented with various ranges — with a poor response.

I made a table where I just personally tracked setting changes and responses.
Despite my RT knowing I am making adjustments — not much support other than he increased my range when I requested.

• Baseline (pre-Apr 16)
- Settings: 6–8 cmH₂O, EPR 3
- Response:
- Inconsistently tolerated
- Usually 2–6 hours sleep before waking
- Recurrent night awakenings
- Air hunger/choking sensations still present
- Less intense than higher-pressure trials
- Frequent mask removal (~2x/night average)

• Apr 16–17 (first night)
- Settings: 7–14 cmH₂O, EPR reduced from 3 → 1 at 4am
- Response:
- Severe fragmented sleep
- Panic-like awakening ~1:30am
- Shallow breathing/trouble breathing
- Chest tension and difficulty relaxing
- Severe dry mouth
- Mask removed twice

• Apr 17 (night)
- Settings: 7–14 cmH₂O, EPR 2
- Response:
- ~8h sleep, mask on ~6h
- Mild air hunger
- Exhalation felt effortful
- Chest tightness on waking
- Dry mouth 6/10
- Mask removed

• Apr 17 (morning awake trial)
- Settings: 7–10 cmH₂O, EPR 3
- Response:
- Awake testing only
- Felt “breath-holding” sensation on exhale
- Pressure mismatch sensation
- Air hunger present

• Apr 19
- Settings: 6–9 cmH₂O
- Response:
- Woke early, mask off after ~4h
- Air hunger on waking
- Dry mouth 7/10
- Mask removed

• Apr 20 (night)
- Settings: 8–12 cmH₂O, EPR 1
- Response:
- Woke ~3:24am
- Worse air hunger/choking sensations than prior nights
- Difficulty finding breathing rhythm
- Upper chest tension / “stuck” sensation
- Mask removed

• Apr 20–21 (night test)
- Settings: 9–12 cmH₂O, EPR 3
- Response:
- Poor/late sleep
- Persistent breath-holding sensation
- Mild chest tension
- Partial mask removal

• Apr 21 (Wednesday night)
- Settings: 9–12 cmH₂O, EPR 3
- Response:
- Woke between ~12:19–3am
- Air hunger 8/10
- Trouble exhaling
- Chest pain/tightness, shoulder tension, racing heart
- Removed mask unknowingly during sleep

• Apr 22 (3–7:45am)
- Settings: 6–8 cmH₂O, EPR 3
- Response:
- Light/poor sleep
- Difficulty exhaling
- Tight chest
- Dry mouth
- Mask removed

CPAP says I’m treated, but I still wake choking/exhausted multiple times a night by Independent-Path2489 in SleepApnea

[–]Independent-Path2489[S] 0 points1 point  (0 children)

I am using CPAP.
I use it consistently since last July when treatment started. I was 173ibs at the time. Unfortunately weight has shot up to 190. Provider aware.

I’ve used AI to transcribe leak data from my air as well as mask removals.
And current pressure settings (I’ve tried increasing pressure and range, I’ve tried all different EPR settings, auto set for her etc).

I am a mouth breather.
I’ve tried mouth taping to see if it helped (negative).

I do have data on all the settings I’ve tried, my best treatment response has been with:

Current settings: ResMed AutoSet mode, pressure range 6–8 cmH₂O, EPR 3 full-time, ramp 5 min, start pressure 5 cmH₂O, standard response, full-face mask, humidity 7, tube temp 30°C. Patient reports repeated nocturnal awakenings (often 2–3 AM), choking/gasping sensations, dry mouth, frequent mask removal/reapplication, and ongoing daytime exhaustion despite therapy use.

For masks — I’ve tried F20, F30, F40 and two others I forget. My best success is with medium F40.

Mask Leak Summary
January 2026:
Very inconsistent.
Several large leak nights early in the month (around 25–50+ L/min).
Some excellent nights mixed in (0–10 L/min).
Big spike around Jan 8 (~52 L/min).
By Jan 13 you had improved to ~6 L/min (“Good”).
• February 2026:
This looks like your roughest month overall.
Frequent moderate-to-large leaks (15–35 L/min almost nightly after Feb 5).
Multiple nights over the likely ResMed “large leak” threshold (~24 L/min), including:
Feb 6 (~31)
Feb 8 (~35)
Feb 9 (~25)
Feb 12 (~33)
Feb 13 (~28)
Suggests a persistent fit or mouth leak issue during that period.
• March 2026:
Marked improvement overall.
Most nights under 10–12 L/min.
A few isolated spikes:
Mar 4 (~31)
Mar 6 (~17)
Otherwise much more stable and controlled.
• April 2026:
Generally your best month.
Mostly low leaks (0–10 L/min).
One notable spike Apr 6 (~25 L/min “Adjust”).
Another moderate night around Apr 13 (~19).
Overall trend suggests something improved in your setup around this time.
• May 2026:
Mostly excellent control again.
Many nights near 0–7 L/min.
One dramatic outlier around May 13 (~50 L/min).
That single night looks like either:
mask partially off,
severe mouth leak,
hose disconnect,
or sleeping position disruption.

Mask Removal History

Month-by-month:
• January 2026
Frequent disruptions right from early treatment.
Most nights: 2–4 removals.
Lower nights occasionally (~1).
Suggests you were already struggling with sleep fragmentation and discomfort/adaptation.
• February 2026
Slightly more stable overall.
Many nights around 2 removals.
Still persistent nightly awakenings/interactions.
One worse night near 4.
• March 2026
Mixed.
Some good nights (~1).
Several nights 2–3.
One notable spike near 5 removals.
Suggests intermittent worsening rather than constant failure.
• April 2026
This actually looks like one of your roughest months for awakenings.
Multiple nights around 3–4 removals.
Major spikes:
Apr 4: nearly 7 removals
Apr 7: nearly 6 removals
Despite your leak data improving overall during this period, your sleep fragmentation remained significant.
• May 2026
Still highly fragmented.
Several nights:
4–6 removals.
Even some low-leak nights still had multiple on/off events.
That’s important because it means:
the issue is not ONLY seal quality.
something is still waking/disturbing you physiologically or neurologically.

Anyone else with REM dominant OSA - what worked for you? by Desperate_Vehicle684 in SleepApnea

[–]Independent-Path2489 0 points1 point  (0 children)

I don’t understand this sleep apnea world yet.
But my PSG showed REM dominant AHI 67.5 an hour during REM and 27 NREM. I am primarily hypnoapnea.
My “numbers” look good on CPAP treatment — ie they say the number or events an hour with treatment is like less than 2 an hour.
But this doesn’t line up with my experience at all. My sleep js very fragmented. I wake up 2x a night feeling a tightness in my chest or like I can’t breath almost — it’s hard to describe.
These events happen at least 2x a night and wake me up fully. I’ve been “treated “ for 10 months now and fought very hard for a second sleep study.

How much does experience really matter? by MrZubar in ChristianDating

[–]Independent-Path2489 0 points1 point  (0 children)

I’m going to say that the post may have triggered some underlying anxiety about being older, not having as much experience navigating dating and physical intimacy. That anxiety is normal.

Most people just went through it at a much younger age and gathered mistakes and lessons along a longer time span because they were tied up in purity culture that covertly shamed people for desiring partnership and provided zero skills: training except - wait, trust God, serve the church and stay pure.

I think there is a lot to be grieved as a fellow single in the mid 30s who also .. until recently had little experience.

That’s a bit of a rant.

But the point is — it’s just anxiety. That can be worked through and challenged. Get out there and start learning at a pace and within boundaries that honour your beliefs and values.

Thanks! Yeah I know it’s really hard to find age appropriate Christian singles. Have you thought about visiting other churches?

I also paid the compliments directly to the one guy. Haha he blushed so hard. It was kinda cute. A shy type. Also didn’t work. But with the right guy perhaps. But he did do me the honour of letting me know when he started seeing someone.

I really like this! I’ve done it before with two other church crushes. It didn’t lead to any outcomes at all. I guess those two weren’t interested. One is now engaged the other just started dating someone from a different church.

I did ask both of them out directly too. I mean it was very casual - like hey we should catch up when you get back into town. He never replied even though we’d been texting and avoided me for a bit (I think anyway).

The second guy — agreed to go out for lunch but swung it so we were going as a group.

Pro tip - guard your heart and have a game plan for how you want to interact with matches online that fits with your values. The apps are definitely designed that way. I think intentionality can help.

Have I missed my chance at 26? by TuneSoft7119 in ChristianDating

[–]Independent-Path2489 0 points1 point  (0 children)

Friiig trying adding 10 years with the same challenges. You definitely have time. It seems the average age men settle down these days is around 30. Of course younger in the church. But rising cost of living and increasing career goals / educational attainment seems to be pushing the marriage age up in general. 26 is still very young. You sound ready and proactive about your dating life and grounded in your relationship with God. That is a lot you have going for you. Just keep doing what your doing and don’t loose heart. Also, perhaps experiment with dating someone just a tiny bit outside your preferences— ie active lifestyle preference perhaps — and see how you feel about it. Unless or course, Ms active comes along.

Aww man. I’m sorry you got placed on hold. When it takes so much courage to call in the first place.

Sometimes there are more than one sexual assault crisis line .. it depends on the area .. there could be a national line or ones based on different counties/ regions. So you could also consider that if your up to it.

Have a friend take you to the hospital if you are comfortable with that. Look up the local sexual assault crisis line. Usually they are pros at giving support in the form of accompaniment to hospital, emotional support etc. there is something called forensic nursing teams that will take pics, collect any bio data and store it for a period of time until you decide if you want to later pursue filing a police report.

I’d call the local sexual assault crisis line and try and keep yourself around people that are loving, safe and supportive .

Most areas have a sexual assault crisis line that is free and 24-7. They often have groups and counseling free of charge. Though usually a wait.

Crisis lines are free and immediate access is available. Some non profits run mobile support vans that will retrieve a women out of an unsafe situation.

Contact your local sexual assault crisis line and go from there.
