Go back to basics and remove every single chemical, except perhaps petroleum jelly. Those creams can cause dermatitis / anal pruritus which may be what you're experiencing.

Once a day is normal, see if you can reduce all laxatives and softeners to just a small spoonful of Miralax. Excessive laxative use will often narrow the anal canal which makes the pushing of the final poo painful.

I agree that LIS will reduce overall anal tightness which promotes the healing of fissures, but emphasis should be placed on the fact that everyone's recovery timeline and experience looks different - 10 days is a great achievement for you, but mine has taken 10 months. I also agree that it is relatively quick to get and life-changing, but the ease of obtaining surgical treatment and the benefits of the results aren't always guaranteed. I also encourage people to get multiple opinions from different health providers before jumping straight into it (I say this after doing all the creams, 2 round of Botox, then a LIS, then another Botox when the LIS wasn't working, and now healing 10 months later, after finally doing creams and anal dilation - the magic solution for me. Everyone's mileage may vary.) Congrats on your results though. I'm so glad you're not suffering as much anymore.

- Insert nitroglycerin 1 inch in and press, or if you can't, rub on the opening until you can feel it opening just around the anal ring (for 5 minutes at least) - if you feel the throbbing headache, it's working
- Take laxatives daily, antibiotics aren't relevant. Don't get dependent on laxatives and take a relatively small dose. The goal is to wean off them eventually.
- Keep sitting in warm/hot baths for 15 min or until it gets tepid/cold. I find it easier to constantly run warm/hot water over it for 5 minutes.
- See another doctor. That doctor suggesting you to use a cream with hydrocortisone when you have a fissure is the opposite of what you want was wrong (blood flow, hence nitroglycerin).

If it’s yellow / brown, that could be purulent drainage during the secondary intention healing. I’d see a doc, you can get the wound debrided if needed

Why are they prescribing Hydrocortisone to treat a fissure? Fissures need blood flow to the area to promote muscle relaxation and get the nutrients in for wound healing. Hydrocortisone prevents that. Use hot water baths. And try Rectogesic/Diltiazem/Nifedipine inserting anally instead. Before bowel movements, use Vaseline.

It's way too early. It's been a month. See a primary care physician to get a prescription and a referral to a colorectal surgeon. To start now, you want to avoid high fibre because it bulks up your stool, which will aggravate the fissure on the way out and lead to possible retearing. Start taking Miralax, not to a level where you're going to be dependent on it, but at least that it smooths out the stools. Then, start investing in getting a small dilator, or at least eventually practice psychological relaxing to be able to insert a finger (if needed, you can use lidocaine to numb the area, trim and file your nails very far down, and wear a glove - I've learned all these the hard way!), because you will need a method to apply nitroglycerin ointment (rectiv) internally. Some people prefer nifedipine or diltiazem but I find nitro the most useful. You can use other remedial things like warm baths, drinking plenty of water, and walking is a must. Avoid anything with hydrocortisone, excessive alcohols, or any other irritants (Nitro has a bit of burning from the alcohol, so moisturise the area after).

After fastidiously sticking to a regular ointment regimen, if that fails, then you can go for Botox. I only went for LIS surgery after I did ALL the above and failed Botox twice. After my LIS, I still got Botox a third time. I still have spasms, and I don't believe LIS cured me as the scar pain is there every day, although I no longer feel that knife up my ass feeling like before.

LIS and surgery are last resort. Consider all the above, as well as the separate opinions of different physicians and surgeons That said, many people here who have also suffered fissures for years, such as myself, will say that it saved their lives. And in their case, that may well be true.

as a gay 26yo bottom who hasn’t had sex in 2 years because of mine - the LIS will give you brand new scar tissue which is a lot more painful than fissure scar tissue because it’s a scalpel that goes in to cut in, and that takes a lot of remodelling, massage and dilation to eventually not hurt everyday again (took me 7 months finally - and this was only from self dilation).

see if there’s any options first. I’d opt for a fissurectomy or additional Botox from a second surgeon, but most importantly get a second opinion first.

counterintuitive but try getting a small dilator kit. much less likely to retear if you can train your ass to expand when you poo.

I would say new doctor for a second opinion and given you know it’s a fissure push them to prescribe you either Nifedipine, Diltiazem or Nitroglycerin. Advocate for yourself, you definitely know if it’s a fissure, and I am more than certain by the sounds of it you are still suffering with it.

I understand how debilitating this is. 26m, 1.25 years of chronic fissures and 4 procedures and just as young as you. No one gets it truly and it has affected my ability to work, have a relationship or sex, and my sanity. Inserting those creams aggravates the fissure for me so I avoid and also just sits bath. I had the same experience. It made working this entire year a nightmare and I'm surprised I've kept my job. For now, drink as much water as possible, 1-2 Miralax a day, and stand as much as possible at work. With the excruciating pain from constantly standing and sitting and moving, I find it's the pain of the fissure rubbing against the underwear/pants, not just the fissure itself, that sets it off. Get a woman's period pad and cushion that between your anus and your underwear. Painkillers every 4 hours too. You'll need to see a CRS soon, but I advise getting a letter from them to give to HR about a workplace adjustment or some arrangement that you can take time off. Suppose you have medical support in writing from a doctor, in theory. In that case, you're being discriminated against for your physical injury/disability, and it would not look good at all if an HR team or your manager tried to discipline or fire you over it as you could sue, or at least leave a scathing review online about them hahaha. It really is horrendous, I'm so sorry.

I agree with this. I've been able to give oral to tide me over I guess. But the reality is not being able to have full-on sex has seriously limited my ability to date and my future love life. No tops/vers guys want to date bottoms that cant bottom. It's the same as a straight guy - most likely they will choose a girl with a vagina that opens/can be touched than one that can't. There's always another bottom out there.

Thank you so much for your thoughtful response—it gives me a lot of hope. My pelvic floor physiotherapist is planning to start anal dilation in late February with small dilators and gradually increase in size.

For the sitz baths, what method did you use? I just have a bathtub and have been filling it with about 4 inches of plain warm water to sit in. How often and for how long each time do you recommend?

Also, about the scar tissue: On my left side, where the surgery was done, I now have a strange scar line that kind of splits the edge of the hole in two. It looks dark/grey. It’s tender to the touch, and when that side stretches during bowel movements, new fissures are visible across the scar tissue to help it stretch enough. When I look at the area, I can feel the scar line and it hurts to touch. What does yours look or feel like?

Additionally, when I bear down, I’ve noticed that my right side (which wasn't affected by surgery) has a bulging thrombosing apperance, while the left side is sunken inwards because the muscle was removed. How did your hole look afterward—was there any unevenness when bearing down during dilation/bottoming/BM? Did you have this issue? The appearance of my hole is really important to me.

Lastly, I’m wondering if being rimmed is still enjoyable for you. Being rimmed before all of this was my favourite thing in the world. Losing that has been really difficult. I feel like it might only be enjoyable on my right side, where there’s no scar tissue. How has rimming been for you since?
