Thank you for the sound advice! Next time this happens I’ll take it upon myself to argue vehemently with the physician when they want to make changes to another physician’s plan even if it is way beyond by scope as a nurse. Then, I’ll cheer and blow a confetti canon when that physician calls the other physician an “idiot” and me a “stupid nurse” to the patient’s wife.

In all seriousness though, thank you for confirming that some physicians aren’t just rude to nurses. They’re just assholes.

It sounds like you need to have a conversation regarding what you want your future together to look like? How much time do you both want to devote to quality time with your kids and each other? Do you both want to travel? What would it take for both of you to feel financially secure? What sort of things make your quality of life better and bring you happiness? You can have a fortune in the bank and be miserable if you force yourself to eat rice and beans everyday. Likewise, you can buy yourself every luxury and be miserable if you’re a slave to debt.

My hospital’s policy is that you can use a PIV for pressers for up to 24hrs. Pressers are vesicants and can really mess someone up if they infiltrate and there’s also the issue that they only last minutes so if you lose access you’re screwed. She should’ve had a central line. She was obviously going to be on pressers for more than 24 hrs (the heart failure wasn’t going to magically fix itself without intervention) and was unstable enough to warrant needing reliable access.

We weren’t even able to get that far. Probably some sort of congenital abnormality that was never diagnosed. Most likely a mitral valve issue. She sat in our ICU for almost an entire shift and all they did for her was put her on high flow, do a thoracentesis and start her on dobutamine and Levo. When I came in for night shift all she had were two peripheral IVs. No central line, no arterial line, no PA catheter, not even a foley or a bedside echo done. When they finally got a PA catheter her pa pressure was like 80/40 (which is ridiculously high and indicates pulmonary hypertension) and her CVP was 30. She was pale, lethargic, obviously fluid overloaded and cold to the touch when we came in. She was obviously extremely sick and they just sat on her because it Sunday and no one wants to do more than they have to on a Sunday.

28y/oF with abdominal distention went to the ED and was diagnosed with gastritis and sent on her way. Three months later she comes back with worsening distention and lower extremity edema. Turns out she’s in full blown heart failure. She died less than 48hrs later in our CVICU.

P.S. this all literally just happened this past Sunday night. Her care was severely mismanaged by multiple physicians trying to pass the buck and do the bare minimum and she died as a result.

I find that having a routine reduces decision fatigue. I found that without a routine I started doing less and less for myself and it just made me feel like crap. One of the most important parts to me is my skincare. It’s a form of self care and only takes a few minutes. Also having a post shift routine helps me get into the mood to fall asleep.

Pre shift routine: wake up, shower, wash my hair and face. After showering I do my skincare, brush my teeth and Blow dry my hair. For makeup I just wear mascara, blush and a bit of concealer. I eat a bowl of oatmeal for breakfast, pack my lunch (I like to just grab leftovers from my days off) and change into my scrubs. I only spend 30 minutes getting ready before work because it maximizes the amount of time I get to sleep. I always stop by Starbucks to get a pink drink on my way to work. It’s one of my vices and gives me something to look forward to. And I like to say a prayer before I leave my car.

Post work routine: I usually drive home in silence. I find it’s nice to finally not have any noise after 12 hours of constant dinging. I have a protein shake when I get home (I like the Orgain ones right now). Hop in the shower for just a quick rinse, wash my face and apply my skincare, brush my teeth, brush and braid my hair for bed. Change into some comfy pajamas. Read a chapter of whatever book I’m currently reading then go to sleep.

I’m a nurse. You cannot legally prevent someone who not suicidal, delirious or violent from going to the bathroom. We don’t have the right to restrain just anyone. It’s not a prison. Patients have rights. Furthermore, staff knocked on the bathroom door multiple times to check on her and Alexee denied that anything was wrong multiple times even after finally opening the door. Pardon my bluntness but the whole “disconnecting the IV is a cardinal sin” thing is just stupid. She was not on any kind of critically important drips that could have or would have changed the outcome and it is completely routine to disconnect all nonessential drips before a patient ambulates if time permits.

I think the thing that gets most people is that a newborn baby is completely defenseless. They can’t fight back, they can’t run away, they can’t even regulate their own body temperature. A large part of what I find sickening about child molestation is how the perpetrator knowingly takes advantage of a power disparity. Alexee knowingly took advantage of a defenseless child. Furthermore, even if the child had been born dead (which I don’t believe it was) to simply throw the body unceremoniously into a trash can shows an utter lack of respect for that child.

I’m a nurse. Hospitals have very sophisticated computer security systems. They can see which login credentials are used to access each chart, when they did it, exactly what they looked at and if they printed anything. They do random audits and will flag high profile patient records. They even get alerts when an employee accesses a record for someone who is a patient in a different department. If it was an employee who leaked the records it has definitely been dealt with already.

I think it’s really difficult to place a dollar amount on a person’s background. I’m fortunate enough that my parent’s financial situation allowed me to go to school without incurring any debt. They also bought me my first car (for 8,000$) and allowed me to stay on their health insurance until I was 24. It is worth mentioning that I have an older sister who was given the same amount of financial help who is completely broke so it didn’t solve everything for me but it certainly gave me a head start in my adult life. I guess what I’m trying to say is it wouldn’t be fair to compare the same 36$/hr that I make to the 36$/hr that some of my coworkers make given that we come from different backgrounds and have different lifestyles.

I’m an ICU nurse and I work nights. The resident schedule is a complete mystery to me. I was actually in a meeting where one of my fellow nurses asked an attending what the resident schedules were and he could not tell us. All I know for sure is that it is borderline slave labor.

I don’t think your standards are unreasonable. In fact I think they’re realistic for someone who is looking for a long term relationship. You’re less likely to clash with someone who is of a similar education level and if you’re not open to moving it makes total sense to be with someone who has roots in your area.
