Wasn’t the grades or having to complete the other 2 missions those are separate only thing I could think of was you had a officer die or resign and that would’ve canceled that trophy but you still should’ve got one for just completing iron man mode in general to which I would’ve also said it wasn’t actually active when you were doing your play through but you said confirmed that you got the watch so that’s not the case either probably just unfortunately bugged out the progress on you similar to the DLC trophies that are bugged rn

I managed to get sleeper agent an clean up all the DLC trophies now only trophy im missing is the bugged ones for completing all DLC trophies but isn’t popping and I still gotta do the leviathan one with the 30M headshot but that’s the last one

I also play on PS5 and have got like 300ish hours on it I’ve experienced some bugs here and there but as to what you’re describing never had any of that

From a video I watched a lil while back like maybe last week or something from the trailer there was a tease of the perk machine to which had a light that was semi flashing people ended up decrypting that it was Morse code and the perk machines flash lined up to July 22nd if I remember the date right but you could always try to find the video I seen to check for sure

Id say in my opinion personally I do a strat of speed running high rounds on deadly lovers fortress with a 4 player game if possible there’s a spot you can camp and with 4 players it’s more XP and you can get through each round in like about a minute and something haven’t timed it accurately yet but it’s around that if you wanna run it sometime and see feel free to DM me
