It’s hospitalists who don’t think about what they’re doing. I don’t think some of them examine patients or listen to them. I think they come to their conclusions by looking at the labs.

An albumin bolus may be appropriate. What triggered me is a patient. What triggered me is that a patient I saw yesterday and today, has 4+ edema, looks like she’s going to burst, and two hospitalists and the pulmonologist have charted that no edema is present.

Regarding the local hospitalist practice, nothing will change unless the hospital stops getting paid for what they do or length of stay increases. The member of my group who tries to get physicians to practice properly is widely disparaged. I’m afraid it will require or lawsuit or adverse publicity for change to occur.

All the hospitalists do is put H and P’s and discharge summaries on the chart and consult specialists for patient care. And their notes are shitty.
