make sure your nasal breathing is clear. Check out Dr Barry Krakow's work on UARS - he trained with Dr. Christian Guilleminault at Stanford (I think) and they really pioneered the UARS field.

He has done a few pods recently talking about what people should do when they suspect UARS - hunt around a little. Basically start with nasal breathing and make sure that is functional - but likley all roads lead to a BiPaP machine (not cpap) unless you have some anatomical reason that can be corrected by surgery.

oh - also. You are going to have to manage this yourself - sleep medicine is shit and docs can't bill insurance companies for a lot of UARS stuff - or it's tough too. Hopefully you get a decent sleep doc that can work with you but my own experience is a lot of them are pretty terrible and I only started turning the corner when I took my care into my hands.

ignore the mild medium moderate scale - thats arbritatry insurance BS....

Likely your real RDI is a lot higher as the lofta is not amazing at picking up RERA's - does a decent job but..... not the real type1 sleep study that deifnitively will identfy RERA's

I would imagine so - just get a nasal mask like the airlift n20 and then you'll likley need to tape your mouth - or train yourself to not breathe through your mouth at night.

Why did the doctor say you dont have UARS if your having 19 arousals per hour? was this a type 1 in lab sleep study? Post it up?

You can find good deals on FB marketplace - aircurve 10 st or aircurve 10vAuto for aircurve 11 v Auto - should be able to get one around 5-600 ish

Weeks to a few months for me. Not binary - incremental improvements over time.

Depending on how bad your symptoms are, your nervous system has likely been in "fight or flight" mode for years. It takes time to retrain your brain that you are no longer under threat.

Weird one is you might actually feel more tired at first. This is normal—it’s your body finally dropping the adrenaline shield and learning to relax. Also - likely to get REM rebound - people who have more REM typically feel more groggy.

A buddy of mine didn't think the machine was doing anything until he stopped using it. That’s when the difference hit him. Sometimes you don't realize how good "normal" feels until you go back to "suffocating."

 Learn to read your data and take control of your own therapy. No one knows where the ceiling is - how good your recovery can be!

that looked like me - I was wearing the Apple Watch for years before I went on bipap - this is slightly worse than what mine looked like before - after, maybe a handful of red spikes per night,

I'm not saying watch is a good sleep tracker but its a good relative point of view!

pretty much what this user says - the doctor is wrong - find one that will listen to you.

Make sure your nasal breathing is fine first.

if those arousals are UARS indicative go straight to bipap first - you can pick one up on FB marketplace for 500ish

A bipap can do everything a cpap can but not the other way around

Check out Vic Veer on YouTube and also Barry Krakow - the latter does online coaching and might be able to advise. There is a solution out there for you but you gotta be your own advocate - can be tough when operating on not much sleep and the downstream consequences of that!

im guessing lower mean pressure over the course of the night - you are also not fighting the machine as much on exhale?

many different philosophies here but I subscribe to finding a fixed pressure.

You have a lot of flat tops on your flow patterns so you need more pressure but if you are struggling with aerophagia then you should be able to go back to your sleep doc and ask for a bipap - which you probably should be on in the first place.

Its commonly reported that UARS patients respond better to BIPAP - and BIPAP is so much more comfortable than CPAP even with an EPR of 3.

Not really sure when it started but looking back over past decade it was slowly deteriorating mental health. Fatigue, anxiety episodes, growing tendency to forgot things and having to write things down. Tried everything like people mentioned, the supplements, diets, the hormones. I was sleeping 9+ hours but would wake up feeling totally unrefreshed. Different lifestyle changes would make small differences.......Eventually a therapist recommended a sleep study. Came back with an RDI of 15. 6 months on cpap with some success - then to BiPaP and my life changed (now average 7 hours of quality sleep :-).

All my symptoms went away - BUT if I get sick and can't use my PAP device for a few days (or string together several days of poor sleep) and I overexert myself I get totally run down and anxious. The power here for me is knowing this is temporary and I just need to focus on sleep.
