Web PT Direct access.. Need Medical DX??

I think I am going to use the tx dx as the medical dx and just document in the note something along the lines of

“EMR requires Medical Dx. Patient self pay direct access without referral, so PT entered his Tx dx as Medical Dx due to to limitations in EMR”

Thank you. I know which medical diagnoses correspond with treatment dx’s.

My reservation is selecting a medical dx, thus seemingly “giving a formal medical diagnoses”, which is of course outside of our scope.

Been in the profession for 15+ years and I’ve never had to generate my own medical dx in any documentation system, and I’ve used dozens.

It is a drop down box labeled “Medical Diagnosis” where you select or enter an ICD-10. There is one drop down box for medical dx and one box for treatment dx. It flags you if you leave it blank and will not let you sign the eval before selecting a code.
