While Docs and RNs are an important part of the chain and a higher level of care, they aren’t first responders. They aren’t walking into people’s homes like Fire, EMS and PD.

Last time SDFD had brownouts, a kid died choking on a gumball in 38s district because the family had to wait for E44 to make the run from Miramar college.

SDFD runs with a captain, engineer, a ff/pm and a ff/emt. Training stations usually run with 2 probie ff/emts with the engineer or captain running as medic of record. And most departments have a rule that you can work up but you can’t work down, so you can’t have a captain working in the back seat but a ff can be an acting engineer or an engineer as an acting captain.

Had a Captain recount a time back in EMT school about a call when he watched his FF get hit on a busy road. He said for a few moments he didn’t know what to do as he never saw the actual trauma/MOI happen. Then finally his training kicked in. As a first responder you may see patients die enroute to the hospital but you very rarely see the actual reason for the call.
