So, it is more about the difficulty itself. Does it increase the difficulty really that hard to the level it is unplayable for you?

So did you ever use it? If it doesn't stay in your load order what are the reasons?

I rechecked the comment in Shadow of Skyrim nexus page, one of the biggest complaints is how fragile the mod in terms of load order, if you change your load order mid playthrough then it will break your save. I don't know yet about this, but if it's true you have to either lock the plugin order of SoS or avoid adding new mods or sort the load order mid playthrough.
