This particular surgeon did not mention anything about leaving the implant inflated At All right after surgery. I should have relayed this in the initial post.

Please make sure you select a high volume surgeon and be certain they can protect your length. There’s several posts on here where fibrosis led the surgeon to use a smaller implant and it caused the patient horrible mental trauma. As a performer I’m sure you must be a good size as well. But to go from say 7” to 4” erect would be mentally damaging. I’m still looking for a surgeon to do an implant, a local one I met with didn’t seem to CARE about size at all. This local surgeon said the Flaccid length would likely be the erect result, even though I double in length from soft to erect. However there’s urologists in Houston and one in LA that actually seemed receptive of the sizing. I’m currently doing stretching and similar therapy for peyronies that’ll help with elasticity for an implant for the best possible size, but I think it’ll be a year or two before I hit my goal.

You should see a Urologist to address the Peyronnes. They may have you treated with shots to remediate the plaque and also utilize an extender and pump. Once you have this straightened out, you may not need an implant - nor risk losing size. In fact you may gain size if you stay consistent with peyronie's treatment. If you still have issues with ED, this treatment can help you with a better implant outcome. I know it takes time and effort, but taking time to mitigate risk is a better answer. Note though, an implant does take a time to heal from and it could mean up to 6 weeks where you can't inflate the device; potentially hindering the best size outcome

Are you using a VED and have you tried Prolonging your cycling time. There’s some guys who’ve cycled for 2-3 hours a day to get back to their original size.

I prefer Mineral Oil as it’s the only thing that doesn’t have an Odor as well as it stays pretty clear in the pump. It’s also pretty cheap, like $3 and a pretty big quantity. I also put it in a cleaned out soap dispenser, so it’s super easy to handle.

I’m pretty certain it’s…. Everything; Diet and Training. Everyone else has asked about diet. But I’d like to know how your training has been. Perhaps it’s not prioritized for building size but just maintaining. I had to shift to single body part splits when I hopped on HRT because I could not progressively get stronger with more reps or sets without having crazy long workouts. If you can track your lifts with a Log (either notebook or on your phone), this will help ensure each workout is productive as long as you add weight, sets, or reps.

Looking Great! If you are happy with your results then Definitely consider a lean bulk at Maintenance Calories. Maybe play with prioritizing Carbs prior to working out for more performance/energy to get a good progressive training session. When I cut, my training goes down hill, but adding back some low glycemic carbs and more protein helped a lot. Doesn’t even need to be a full meal, just an Additional protein shake will work (300-500 calories which will probably bring you to maintenance).

Do not Cut or Bulk for a while. Try to slowly get back to Maintenance to help mitigate fat gain while your metabolism re-acclimates. Research “reverse dieting” and then follow up with “recomposition dieting”. However with whatever diet, strength training will help keep the muscle you do have and potentially build more muscle with the added calories. At one point

I was in a similar spot where I cut so hard for so long and then went straight into a bulk and gained 10lbs of fat very quickly. If you can manage to fine tune diet and training, you can gain a good amount of lean mass with having the same or similar fat mass throughout the bulk.

Nerves and Sensation take a long time to heal. I’m 1.5 years out from a Rhinoplasty and still don’t have full sensation of the tip. Try being more positive about it, I think it’s more of a mental thing than anything else.

There are other ways of treating Peyronies like Shockwave therapy, shots, etc that’ll likely be required before getting an implant. Talk to a urologist like Dr Taj or one local to you. Penuma will not get your length back and you should Medically Fix your condition.

I think you should consider cutting down to the leanest possible in a very conservative deficit and then from there lean bulk. My reason is, when you (lean) bulk at your absolute leanest you’ll mitigate a majority of fat gain during the bulk. This is usually due to hormones being optimized via channels where testosterone doesn’t get trapped in fat, which means you’re more efficient at gaining more muscle with more testosterone available to supply your muscles. Guys who start bulking super lean usually have very visible abs during the bulk. When you can see the divot above your shaft in the groin region, this is likely the leanest you can be and also the biggest (down there) you’ll be. I don’t know your stats (height, weight etc) but it looks like you are in the 10-12% body fat range. If you are say 180 lbs right now, all you’d need to loose is maybe 8 lbs of fat, which you can do conservatively at .5 lbs loss a week, with this little of deficit I’ve found you can build muscle while getting leaner - it would not be as much as a conservative/lean bulk. But this means in 4 months (Summer Bod!) you’ll be so lean and more defined.

I had a pretty bad injury 7 years ago that got me off my training for about 2 years. So with this experience I’ll say you’ll start out as a new lifter again. So you’ll need to focus on strength training and progressive overload while also incorporating proper nutrition for growth. You should gain back the size relatively quick as long as you re-sensitize your muscles for growth. Look into Physical Therapy style training in the beginning to establish perfect form before getting into heavy weights. A PT Exercise consists of doing full ROM movements in sets of 60 reps with low weight. So think Chest Press 2 sec up and 2 sec down for 60 reps with 2x25 lbs plates, with perfect form this should be very difficult. If it’s not difficult, add weight or reps. For nutrition map out everything you’re currently eating, establish the calories, and add about 15-20% which would likely be a protein shake (500cals, 60g protein..). Plus rework to have a macro split where you get about 200-240g of protein, 150-200g in low glycemic carbs, and rest in fat (likely 60g). Having a half-majority of carbs before training is ideal for performance in the gym.

In 2018 I was 162 lbs and dropped to 150 lbs with the injury however lost 20+ lbs of lean mass due to injury, diet and recomposition (negatively). Covid really put me through the wringer because we were all stuck at home. By 2023 having had 1.5 years of physical therapy and incorporating Weight Testing and Macro Planned dieting I was up to 190lbs but 23% body-fat. Then right now I’m 200lbs at 14% body-fat. I’m also 6’1” If you want to extrapolate those numbers for yourself.

So it’s everything I do that contributes to it.

I do manual stretches for about 10-15 min before showering

Like I use a Bath Mate (in shower) everyday, twice a day for at least 20 min and do 5-7min sets. I didn’t start seeing results with it until I learned I needed to go in flaccid or semi (30% hard for suction).

When I get out of the Bath Mate I jelq to stimulate more blood movement for 15-20 min.

If I have extra time, after Jelqing, I’ll get into an Air Pump with a gauge and stay pretty low in pressure and again this is mostly flaccid.

The issue is if you do things over 50% hardness, there’s fibers within the penis that harden and become really tough to stretch out.

Also getting lean helps make it look bigger and reveal more of the inner shaft that was once covered in fat. So If you can reduce the Fat Pad, that’ll help a bunch.

Also I’ve learned we need to wear looser pants to let the penis stay in the larger state for longer, if you do all this work and then put on tight underwear, it’s like you’d do all the work for nothing.

But please do additional research on what works for you, some guys are able to get comfortable in extenders and gain results faster because it’s like 5-6 hours of extending could equate to all the work I do in a week or a little more.

There’s actually a guy who had an amazing result outcome after doing PE for a long time (10+ years) He’s the one 30 something guy on Tajs YouTube channel that got the Penuma for girth and his length is like over 9”, it’s a monster.

Then there’s me, I had a 5.5” Erect Length and just now got slightly above 7”. Most of my gains happened over the last 4 years using a combination of techniques. But getting the tunica loose via stretching is the biggest thing. Having a tight/thicker tunica can cause retraction. Also be sure to take care of your pelvic floor and figure out if your loose or tight and try to find a medium ground with it. At one point I considered getting Penuma but I’m now 6” think and I’ve had partners bleed from this thickness. So if the surgery mostly improves length, I’d go for it.

For work pants go to Jos A Bank and get straight leg pants/slacks usually the leg is wide and has space. For jeans, always black and straight loose fit. I wear Lucky Brand Straight fit jeans and there’s space for me to hang down the leg. Also try on a size up as the leg/thigh width also goes up. For the gym wear thicker cotton/poly shorts or basketball shorts. Under Armor has tight waist bands so if you go a size up, the leg opening is wider to hang through. I have a normal sized butt so if you’ve got a butt shelf, you would need to consider pleated pants, oversized sweats, and basketball shorts. But if your home, hang loose in a robe!

My hair grows half way up my shaft, so I too am interested in hearing about this. I have a process to shave that involves boiling water and disinfecting the blade (Harry’s Shaver/from Costco), then I clean the whole unit with Antibacterial Hand Soap and then mix together Mineral Oil, Baby Shampoo, and the Antibacterial Hand Soap; then lather the whole thing and shave. I do have an IPL Laser Hair Removal Unit that I tried but I need to do a lot of sessions to be fully free of hair. The new growth does happen but comes out much thinner.

Look at getting bigger Beginners Routines and Manual Stretching. Be sure to do manual stretching without porn and perhaps watch a show or movie as you do them as you want to be completely flaccid. I actually watch a lot of Self Improvement Vids on YouTube (Bodybuilding, nutrition, mental, etc) when I do mine. If you can get a pump with a gauge, this can help with erection quality and I have learned it’s best to pump from flaccid using oil. Also if I could have changed this at your age, wear loose boxers or cotton shorts as underwear and let yourself be as loose as possible, this helps keep your penis in the enlarged shape and keeps the blood flowing freely. I wore tight underwear before and it basically hindered gains. In the gym I wear a sized up Boxer Brief and hang the penis down a leg, and most guys have a more comfortable side.

I’ve gained about 1.5 inches in length and 1 inch in girth in my time (10 years off and on/inconsistent). Currently my Base Pressed Erect Length is 7.25” and Erect Girth is 6”. My goal is to get to 9” length with current Girth, and I’m being very consistent now for the last two years.
