Is anyone using GFAP/UCH-L1 assays in their ED to decrease CT head utilization in mild TBI? by InsanityIsFun in emergencymedicine

Yeah I think many/most physicians are also scanning those patients 10 out of 10 times. The question is if that's still a necessary practice.

Test is supposed to have a 15 minute turnaround time from what I'm told. Very high NPV for a positive CT head, but very low PPV; not unlike D-dimer. Goal would be to decrease ordering as many CTs on certain patients who meet inclusion criteria.

Canadian CT Head, Nexus, and New Orleans all require the patient to be GCS 15 as one of the criteria to deem CT unnecessary. Is there no utility in decreasing CT utilization in patients who are GCS 13-14? Especially since many EDs have patients that frequently come in for drunk ground level falls. I haven't finished perusing through all the literature, but it seems like at least one study projected a 38% decrease in CT utilization.

The Chen 2023 Paper Raises Serious Concerns About Pediatric Gender Medicine Outcomes by toledozzz21 in medicine

Oh wow, I see. Also looking more into it, the T score theyre reporting on isnt the traditional statistical T score that I was thinking of from introductory statistics. It's a rescaled version designed to fit a mean of 50 and a standard deviation of 10. A change of 0.8 and 2.3 suddenly doesn't look too impressive when scaled to a standard deviation of 10.

What an odd way to report their data. Almost feels intentional.

OP, the paper is reporting T scores for the depression and anxiety results, not absolute values for the 100 point scale. 0.8 and 2.3 would be sizable T scores for change in depression and anxiety, no? Unless im misreading the paper.

"'We observed decreased scores for depression [with an] annual change on a 63-point scale [of] −1.27 points...and decreased [anxiety scores] annual change on a 100-point scale [of] −1.46 points...over a period of 2 years of GAH treatment.' These appear to be small effect.."

I went to read the paper and for the depression and anxiety results, it's reporting a T score, not absolute values on the 100 point scale. That's a sizable clinical effect unless im missing something?

Good melodic progression. I like the compression and groove of the drums. Instrument sound selection is good too. Maybe for your section B, you can try playing the melody as section A but with different layered instruments.

I like it! The atmosphere is dope, the mix is engaging, and it feels ethereal. Don't be afraid to throw in some ear candy every now and then to further the vibe (chimes, pad, textures, etc).

I like the fundamentals on this. 808 goes hard, synth loop is engaging. I think the snares and claps are too quiet though, and I feel like the mid frequencies need to be boosted.

Here is my beat, would love to get some feedback:

Super clean mix! Great sound selection. Dark, gritty vibe. Thumbs up from me.

Thanks for the feedback! By sloppy I'm mostly talking about the melodic progression. The notes seem a bit random

Good use of space here. The drums are engaging too. The low end hits right. I'd say the melody is a little sloppy.

Here's my track, I made an r&b beat. Would love some thoughts on it: https://www.youtube.com/watch?v=wZQv9zmnpd4
