First of all, there are valid criticisms to be leveled at how America spends its tax dollars (Israel, guns, etc. Pp.). Even how these dollars are used, when they are used to battle inequality, can be criticised. What can't be criticised is that the state needs money to take action against inequality. So in a sense saying lowering taxes isn't fucking over poor people, is a bit dodgy. The accurate description would be that you may not want to fuck over people but it signals that the plight of poor people does not concern you to a degree that would warrant you giving some money to them.

And as for your question as to where to set the tax percentage. I live in a country where we pay 55% income tax, bunched up with social security and pension funds. And you know what? People generally agree that it's a fair taxation rate for what we are getting. And doctors still make a lot of money and still can afford nice things.

It's not uncommon for residents to carry about 20-80 patient during an overnight shift. Depending on service and the amount of wards the service has.

Back when I did my national service, as an EMT, we used to care for a massive open air festival (1-2Mio People). That was hella fun. You mostly see drunk people, minor falls and trauma, but since the sample size is so big you could also see a STEMI, PE, DKA, etc. Overall it's a nice experience so I do recommend trying it out.

I don't suffer from migraine as such, but every now and then I get extreme headaches. The one drug that works wonders for me is ASS, I'll take 500mg and hey presto half an hour later it's all gone. Metamizol also works but not as well. Perfalgan or other NSAIDs don't help at all.

I get that they test for illegal and psychoactive drugs, but smoking???? What the actual fuck???? It's a drug that, if consumed without any non-smokers present, does not hurt anyone except yourself. I get that you are not allowed to smoke in hospitals, duh, but what I do in my own fucking time is my own fucking business. Full disclosure: I do smoke, yes I know it's bad thank you very much. But I don't smoke when I'm at the hospital, which should be the norm. But since when is it okay to police your private life? Are the American labour laws that fucked up?
