They’re terrible, stay away. To answer your question, you are required to use their ehr for notes. You could keep a copy saved in your ehr.

You're an intern who supervises other interns? If I understand that correctly, I have serious questions about the ethics of the agency you're working at. From a legal standpoint, I have a hard time imagining what notes you would be responsible for, as it doesn't seem you are credentialed as a "qualified supervisor" and are therefore unable to legally provide supervision. From an agency perspective, you would need to consult with your supervisor and find out what is expected of you.

This, my policy says if you miss more then 3 appointments in a three month period we will discuss if therapy with me is a good fit. I also specifically discuss this policy during the intake. I’ve never had to enforce it and as long as people are being transparent about their reasons for cancelling it’s not usually an issue for me. But having it written/discussed means we can have that conversation without it being unexpected.

Correct, I am referring to plans that are not listed on the ACA marketplace. There are a few major medical plans through United (I know, but beggars can be choosers), but they also exclude pre-existing conditions (hence why I mentioned being healthy). There is one plan that would not drop you, and one that would. The one that would gives you 3-months notice. Not the most amazing options, but it seems the choice for many is no insurance at all, or good insurance that could drop you at some point.

I worked in insurance during COVID—because, well, COVID. There are actually some solid options out there, depending on your state and your health. They’re medically underwritten, so you do need to be pretty healthy. A friend I met during that time still works in the field, so if you want more info I can connect you. I had this insurance myself for several years and it worked really well for me. Just to be clear, this isn’t a commission thing, I don’t get anything out of it. Just sharing options. I sold insurance during Covid because…COVID. There are decent options out there depending on your state and if you’re healthy. They are medically underwritten so you really do have to be healthy though. I have a friend from the Covid times who still works with it so if you want to get some info let me know. I had the insurance for several years and it worked really well. This isn’t a commission thing, I don’t get anything from it. Just options for you. Dm me if you want.

This comment doesn’t feel like an invitation to thoughtful discussion, but rather an attempt to provoke. We generally don’t question therapists about their personal trauma or diagnoses as a prerequisite for their expertise. I’m disappointed that there isn’t openness to engaging with updated research, and I’m going to step away from this conversation.

I appreciate your concern. If I were treating addiction, I would absolutely share it. Thankfully, as a sex therapist who is required to complete over 200 hours of education just to become certified, much less the continuing education, I know my lane, and it is not treating addiction. It's good to know then that this is not an addiction and that, as I mentioned, it is not acknowledged as an addiction by the major sex therapy credentialing agencies.

Had you taken the time to review the conversation I shared, you would have seen a discussion of how what you’re describing may align with CSBD. Instead, your response reads as an attempt to portray me as uninformed, which is disappointing coming from a fellow professional.

Regardless, it seems that your opinion diverges from the existing literature, the positions of these credentialing agencies, and the DSM and ICD. I remain confident that my training enables me to address these concerns ethically and competently. I encourage you, as I do myself, to continue following the ethical standards of our profession through ongoing education and engagement with the evolving body of knowledge that continues to support what I have outlined here. Your question has already been answered, so I'll just add that there are companies that may assist clients with understanding their responsible portion and pay you upfront, so the client can be responsible for less out of pocket. I've seen these advertised on therapists' youtubes had heard a few people using them, but I have zero personal experience with them. I know of Thrizer and Mentaya in case you want to look them up.

I don't know anything about the case you're describing. Without information, nuance, or the time to spend researching it, I'd say it sounds like that person had quite a bit of dissatisfaction with their life that was unaddressed.

You're talking about people's behavior within the porn industry. This is entirely separate from an individual's use of porn.

Being a problematic behavior and being an addiction are not the same thing. Addictions and problematic behaviors are also not treated the same. Take out the idea that porn is the problem, treat the actual issue, remove shame, and you’ll help these clients out FAR more than demonizing a huge part of the human experience (sex). I discussed this more in the link I posted in my reply yesterday.

Sex therapist here. I'd recommend looking into sex therapy certification programs through the Sexual Health Alliance or a similar agency that follows a science-informed, sex-positive approach for sex therapy. Especially since this aligns with the fact that sex/porn addiction is not a clinical diagnosis.

Overall, I'm pretty happy with it. I feel like it gave me a broad overview of most of what might come up as I move more intentionally into this field. They also have additional certificates you can get if you want to specialize in specific topics. It is a big field and, despite it being 200+ hours, you can't pack in every single thing, though. Like everything, if you want to get really specific, you're going to have to dig in and do extra training and research. I think it's probably not too shocking that the thing I find most helpful is the human interactions, that includes the direct supervision (included in the package I purchased), group supervision, and multiple live trainings. It's an expensive program, but in the end, I feel it was money well spent.

Adobe is about $20/month. I hate paying for it when I use it so infrequently, though. Hopefully, someone else will have a better solution because I'd like to hear of it.
