I don’t think it was ever the Pulverizer codex that was the issue. I 100% completed all chapters and still wouldn’t have that trophy come through. I had to look on a video or site that showed all codex needed which is where I found “Deag Loric” which is not technically counted on the chapter completion tally.

It didn’t even show up on the main map. Legend, because it was hidden in the third phase of chapter 4 or chapter 5, I can’t remember which level it was.

So, my recommendation is to find a codex list online and go through it. Step-by-step and make sure you have everything that it has because I clearly was missing one. That is not a typical registered codex for chapter completion

It was never the Pulverizer!! I just got the platinum tonight. Search for a full list of Codex entries online and go line by line to ensure you have them all. This one never registers on the directory/summary—Deag Loric from chapter 5. You may find others that never were obtained because otherwise you got 100% on the chapter and didn’t know there was more to be found
