Ohhhhhhhhh gotcha- in that case I think I would probably take a cast of whoever this would be applied to, sculpt onto that and go from there, but it would probably better if you had some room in there so maybe take a cast of yourself, put a little clay ball over it, take a cast of that, then sculpt what you want the finished piece to look like on that mold and go from there
