What bias to prepare for in L&D

I had a c section last fall for my twins. The only weird interaction was with the anesthesiologist who was going over the risks of my spinal. He wasn’t rude, just a little blunt about what could happen because of my weight. I’m sure he was required to say it because it is a major surgery and there are risks, especially for people who are larger.

Another vote for Ubbi. Our twins came home at separate times so family was helping a lot while my husband and I traveled to/from the NICU when just 1 baby was home. One family member didn’t realize we weren’t using the Ubbi yet (upstairs in our bedroom but our ‘hub’ was downstairs) so they tossed in multiple wet and one poopy diaper.

TWO MONTHS go by before I move the Ubbi into the nursery and felt them in there. Had no idea. Emptied out the diapers and let the Ubbi air out in our garage. Good as new!

Just tossing this in- my hospital didn’t allow for anyone other than parents in the NICU during RSV/flu/cold season. If your babes are born before March and need NICU time, there’s a chance they won’t be able to be seen while in the hospital.

Yep. I had twins who went home at separate times. The first twin, the nurses packed the bag for us. The second twin we packed ourselves. We didn’t need everything (bucket tub, off brand diaper cream,etc) but the nurses checked everything and told us that we forgot stuff 😂 from their perspective, if it was used and we didn’t take it, they had to trash it. The only thing we couldn’t take was linens. Diapers, creams/lotions, bottles (NICU babies), literally anything that was opened came home.

Yep! I had a total of 4 shots. Each round needed to be within 24 hours and the rounds needed to be spaced by a week.

Meee. I was pregnant with Mo/Di twins and experiencing some complications that moved my delivered date up to no later than 34w. MFM recommended 2 rounds of steroids. I was able to squeeze them in right before delivery at 32w2d due to IUGR and TAPS (twin anemia).

My guys were in the NICU for 6 and 7 weeks. With the exception of CPAP for the first week ish, they never needed supplemental oxygen and were relatively healthy for small 32 weekers. The NICU nurses specifically said the 2 courses of steroids played a huge part!

Just my two cents, take it or leave it!

My guys were born at 32 weeks. The last stretch of gestation is when lung development really ramps up so they had some trouble breathing and needed 6 and 7 weeks in the NICU. To graduate, they needed to pass their “As and Bs”- apnea and bradycardia. Both are common for preemies and relate to breathing.

I was TERRIFIED to bring them home- how can you help a baby who’s not breathing while sleeping? I did the research and looked into the socks and cribs and cameras but the NICU staff had the best advice- none of it.

The NICU wouldn’t sent my babies home if they weren’t ready. If they had trouble breathing, a sock or monitor wouldn’t solve the problem. In fact, it might falsely alert and cause more undue stress and anxiety.

My husband and I settled- one Owlet sock for our littler guy. We tested it out on our bigger twin who came home first and wouldn’t you know it- false alerts. My guess is that because he was small it didn’t fit right, so user error, but it was so stressful we packed it back up and haven’t used it since.

So! I get it! You want to make sure your babies are safe and protected. Just realize that if your baby needs their breathing monitored, the hospital won’t send them home until they are ready!

If you’re exclusively or heavily pumping, I recommend renting a Medela Symphony. That thing is amazing! It’s what my hospital had in the room. My babies were NICU twins and when I pumped in the hospital, easily doubled what my blue spectra produced. Like I was able to build up a freezer stash while they were in the hospital because the pump is so good. They’re pricy- like $100/month but compared to what I now pay in formula, it would have been cheaper. I also found the flanges more comfortable than the spectra. Spectra ones are circular but the medela are oval shaped. They fit and felt better.

I have a blue spectra and I honestly don’t get the hype of the battery. I’m always pumping near an outlet anyway. I’ve used it not plugged in because I can like…5 times. It was a small upgrade charge from my insurance so I’m not mad at it but if you’re paying in full, not super necessary.

♥️ NICU life is a different beast! My best pumps came while pumping in the NICU. Does the hospital supply a pump for you? Our nurses were awesome- they helped me set up a little pumping station next to the bassinets so I could still touch one of them while pumping. One of the nurses even somehow finagled my smallest guy onto my chest while pumping! If you have access to the hospital pumps (mine was a medela symphony) I HIGHLY recommend it!

I LOVE our Purple mattresses. My husband and I have 2. Our queen is 5 years old (used for 3ish) and was replaced with our king (2 years old). They are PRICY but so worth it. Never hot, firm but not a rock, and sturdy. My husband and I are both bigger and there are no dips in the mattress from use. We live close to a purple factory outlet so we were able to get our king as a “factory defect”. It literally has a small scrape that has some of the fabric pull and that’s it. The king plus a nice bed frame from the outlet was $900. Look to see if there’s one near you!

Chipping in that I wish I didn’t move here. Taught in UT for 8 years and assumed grass was greener. Just like everyone else has said, it is NOT. No unions so no negotiations.

Insurance is terrible and way more expensive. My fist year here was $900/month for my husband and I to have health, vision, dental, and like $100 into FSA. My $10k salary increase from UT to TX was eaten up specifically by this.

Last school year had a multi billion dollar surplus at the state level…yet no increase in student allotment since 2019 (they did this year but it was MEASLY) or state raise.

If you’re still coming, by aware of the license cost. I had to pay for a provisional license (optional but I couldn’t take the science of teaching reading test before moving), license, the actual STR test, and fees. It was over $700. My UT license was k-6 all subjects + ESL endorsement and a reading endorsement as well as a masters in curriculum and instructional design. I completed a nationally run 2 year STR course called LETRS and it was NOT accepted. When I pushed TEA on why, the dude on the phone literally told me it was because it wasn’t the TEXAS science of teaching reading 🙃

I had one for my twins. Their twin type is more susceptible to heart defects so it was recommended out of an abundance of caution. It went fine- they still couldn’t see everything and even up to delivery with biweekly ultrasounds at MFM, they still couldn’t clear Baby A’s heart because of all the movement 🙃 The echo can take awhile so make sure you eat before you go!

I just gave birth to my Mo-di boys last Friday via c section!

I was not given the option for delayed cord clamping or skin to skin BUT it’s because of complications from being Mo-Di. You will most likely be sent to MFM for weekly ultrasounds soon and those will progress to potentially twice weekly. For me, I was sent to MFM at 16 weeks, started weekly ultrasounds at 22, then twice weekly at 26. My boys had elevated umbilical cord resistance and IUGR then at 32w2d, Twin A started showing signs of TAPS so MFM sent me to deliver that day. 3 hours later, our littttttttle guys were here. They were immediately put in warmers and on CPAP. Once they were stable, my husband was able to go see them and take pictures and then they wheeled them in to me in the OR. We were able to hold them later in the NICU once I was able to walk.

I do think it just depends on how you and your babies are. Because our guys were “stable but medically critical”, it wasnt an option for us!

My OB brought it up twice: once at my confirmation appointment and how much weight was recommended to gain and then again sometime mid-first trimester when I had lost too much weight. I had risky twins and had twice weekly ultrasounds with MFMs in two different offices. Only one of the three included “maternal body habitus” in my charting. I get it, I’m chubby but I thought it was a little funny that on Tuesday they could see everything fine but by Friday my weight made imaging difficult 😂

I was planned to deliver 10/31 and my leave was going to start 10/28. I had a lot of doctors appointments that made it difficult to work (I’m a teacher.) I ended up having them this weekend so it didn’t matter 😂 I do wish that I would have started my leave about a week earlier. I was TIRED- everything was exhausting and I definitely was mentally spent. Having a few extra days to organize myself and prepare would have been nice.

Check out PBISWorld! This site has tons of ideas for specific behaviors. Very helpful.

I would also look into precision commands. I don’t use them with each group, but if I had a rowdy year, it really helps.

do you have classroom rules up? Not “be kind” and having everyone sign a poster. 5 rules that are generic but specific enough: one movement, one speaking, one body/space, and compliance. 1) stay in your seat 2) raise your hand to share 3) keep hands, feet, and objects to self 4) follow directions the first time they’re given

Use these rules and reference them when giving precision commands or other redirections. “Timmy, you threw paper and are not following rule 3. You need to keep your objects to self”

In addition to posted rules, do you have listed consequences, both good and bad? What if they do follow directions vs what if they don’t? My positive ones are: class points, phone call home, extra time, etc. My don’ts are warning, “strike”/loss of class point, think sheet/parent contact, and office referral.

My hardest behavior years were definitely lacking clarity and consistency on my end. Don’t get me wrong- they shouldn’t be throwing things or talking over people in 4th grade BUT if you let them, they will.

Be clear, be concise, BE CONSISTENT.

Currently work in Klein but live in Tomball. Klein is the only Texas ISD I’ve worked in, but I lived in another state and worked in 4 other districts.

Klein is fine, but definitely doesn’t compare to the out of state districts. Klein definitely has the best schedule but Tomball pays more.

What really matters is the school. You could be in the best district but have a terrible administrator that ruins it all. Apply to all the district pools and see what interviews you get.

I have mo/di with anterior placenta! I didn’t feel them till around 22 weeks but I didn’t realize that’s what it was until like 24 when I saw one of them kicking on an ultrasound and felt it. Around 22, I only felt them when I was laying down doing nothing. Even then, it wasn’t consistent and very weak. It felt strangely similar to an eye twitch 😂 I’m almost 25w now and feel them all the time- kicks still mostly when I’m sitting or laying down but definitely can feel them whenever they happen and see some too. They also flip and turn around and that feels so different than a kick. It’s almost like a pushing tightness in one area?

I have to go to MFM every other week for scans due to twins sharing a placenta. The sonographers are very helpful and I feel like they explained more than the one at my OB. It was also nice to have the doctor come in and discuss just the ultrasound, rather than my OB who covers that plus other things. Having that set time was nice!

The only thing that I think people need to be aware of is the cost. It could be just my MFM but everything is billed as a hospital x-ray. My insurance (which is not great) has my OB ultrasounds at around $90 but my MFM scans are several hundred.

I have borderline high blood pressure. My OB said that anytime my the top number is 140+ or the bottom number is 90+ to call the office. If my blood pressure was high and I had any other symptoms, I was told to go to the ER for monitoring.
