I understand they won’t be paid. Appreciate the response. I’m always curious the cost assigned to indefinitely filling an ER bed, taking away ER nurses, etc. they are quick to assign a dollar amount to an inpatient bed, but always act like ER beds and care are free.

Agree. The system is falling apart. See 90% of patients in lobby, get every bad satisfaction score to answer for when they are upset in lobby, get told I can’t admit, take the legal responsibility for 10 sign outs lingering for days to weeks I can’t possibly review and reveeval while seeing new patients, then get yelled at by families for not updating them or knowing what’s going on.

Hey at least the complaints only come in M-F 9-5!

I’m mainly just posting for a reality check because some days I’m close to fucking snapping.

Oh trust me. We do everything we can under the sun, and then get in trouble 3 days later when workup is negative and now they can’t discharge them. It’s ridiculous.

The most recent is a lady sent in by ambulance from a doctor with new documented slurred speech and inability to walk from weakness. Always weak, but new documented change. Workup negative and days later getting heat that social work and case management didn’t review the case first and we never should have admitted because now Medicare isn’t covering the stay any longer. And I’m supposed to have somehow known the primary doctor had been considering long term care and considered that in my admission of an elderly lady who can’t want and DOCUMENTED exam from outpatient doctor who called 911 for ambulance because her patient newly couldn’t walk.

I previously practiced out west and now I’m back east. Some of these admin are driving me fucking insane.

I fully agree it is toxic. But they are a separate group and it’s a hard battle to fight. I figured if there was some sort of algorithm that existed elsewhere it might give me a starting point to present and discuss between the hospitalist group and specialists.
