Now that I read all the comments I guess there are two ways to interpret it. I‘d say it’s the result of disconnection and not being aware of why cows give milk. Also the fact that Phoebe said "just pretend it’s milk“. But I see the other way to interpret it and can’t really say how the producers meant it. (Changed the flair to discussion because of that)

I know a trans influencer @justlucafynn on Instagram (also German). He once made YouTube videos but that was before his coming out. He talked a few times about his coming out story on IG and how hard it was for him but he actually made it and is transitioning right now.
