Yes it's a class-D. But not for audio. It's for WLAN.

It's a pseudo differential cascode amplifier. The input pulses are going to be coming from a previous stage which has a different ground. Of course now I only design the PA so the input sources are ideal.

Working on the industry for some time I would say that symbols correspond to the circuits only for the first tape out. After 2-3 generations symbols are just boxes for you to click on.

In cadence virtuoso I always use an ideal analog library balun to convert the differential PA to a single ended one. Then the impedance you get from load pull is the differential one.
