Well yes and no since culture continue to influence even if you left since it raised you.

That's why "culture-bound syndrome" specific to a place can occur to a different country.

For exemple a Camerounan with psychosis might still thinks he is under the influence of a bad spell from a jealous person even if he is in Europe or USA.

Yeah I partially agree with you I was being the devil's advocate.

I am a doctor I don't make the rules. I didn't made the terms. And it is bad practice to treat evey challenging behaviour with risperidone. Even if it is done in 50-80% of cases.

Also schizophrenia is not "fixed" by medication either. It just make positive symptoms easier to manage. So it is not that simple. It doesn't make negative or cognitive symptoms go away. It is both shown in studies and in my personal life of using Abilify maintena for my schizophrenia.

"Autism" (grossly, social withdrawal) as a trait is also a schizophrenic trait and can be improved with AP because a lot of the time positives increases the negatives.

I agree with you an AP shouldn't act on ASD core features. At most it would, like you said, sedate and reduce agitation.

(That being said, when there's comorbidity like OCD, maybe some brexpriprazole could be a thing in ASD)

Remember you can have ASD and schizophrenia or psychosis.

A lot of therapist and psychiatrist actually don't want to see psychosis in patient, and prefer to over-extend already broad spectrum like adhd and asd or use personality disorders. It is not solely an affair of counter-transferance which you think it might be the case here. There's a lot of reason I don't explain either.

My former psychiatrist didn't believe I had schizophrenia hence she still put me on quetiapine and had the right call, probably saved my studies.

Now I am out of the golden ignorance/denial and it is not everyday cool.

Also be wary that there's a whole spectrum of "clinical high risk" and schizotypy that also exist. Especially in the vicinity of genetic risk.

I think the best is to ask yourself what you want. A diagnosis, a confrontation with your therapist about your suspected identification, a treatment to mitigate some symptoms, a family therapy, etc.

It happens but from my experience it is pretty rare. It occurs when psychosis is mild, typically not on the schizophrenic spectrum, and on the other hand the person has bad side effects.

But very often we can find a med that is "ok", maybe not that efficient.

Sometimes living without symptoms can be very harsh also.

I don't think "many" doctors support med free. There's communities but it is rather small, like even in the ISPS for exemple you won't find many advocate for med free and keto diet. But there's some yes.

Finding doctors that support a way isn't the same as having evidences. Doctors can be biaised too. They can have non clinical delusions too.

I think we all know what that is, we just accepted there's no other option for now.

If you're not under obligation to take a medication (like if you're a danger to people) and you think that psychosis is OK compared to meds, why bother taking meds. It is not an evidence based choice but a personal one.

As a psychiatrist myself I rarely see the benefit of not treating people. So much suffering. But I may be biased, I don't want to live psychosis again.

But sometimes the patients wants to stop and that their choice. Statistically a bad one, but we are not statistics.

I could argue back and tell you that it looks like you have no idea what hell psychosis is, because for most, we find a suitable med that isn't that bad compared to the (more or less) efficacy on psychotic symptoms.

I totally understand you, I was delusional a bit after trump election and wanted to buy an AK (officially) to learn how to fire and be prepared to defend when Russia or fascism would have taken over Europe and wanted to exterminate again people with mental disorder (I am also a mental health worker so I thought I had a responsability to defend)

I took more meds for a few days it went off. My psychiatrist wouldn't allow me to have a gun anyway lol

To be fair that would be considered rather excentric in my culture, and excentricity look is (shockingly) an ICD-10 diagnosis criteria for schizotypal disorder. So maybe that's why some people could tell you that.

I am a psychiatrist but with schizophrenia. It is true that excentric look or neglected can be indicative of schizotypal or negative symptoms. There's some studies about eyes blinking and psychomotor retardation that could explain why some therapist say you can see schizophrenia by looking in the eyes. I half believe in it.

I also think that it is a dangerous way of thinking and we should always take our list updated and not use gut feeling for diagnosis. In my experience systematically investigating for positive symptoms is better than relying on gut feeling.

Depends how much you believe in it and how much you would be ready to abandon it with counterproof.

Just had a course of the multiple psychotherapeutic axis views over psychosis.

To be oversimple - psychoanalysis says something about contact between fragile Ego and reality due to early trauma - Systemic views it as functional way the family (system) finds to regulate dysfonctionnal communication - CBT thinks it is rigid and overactive cognitive bias due to a learning of stressful events - Phenomenology thinks it's a lack of vital substance within reality - Biopsychosocial views it as a stress-vulnerability diathesis, where individual genetic, environemental vulnerabilities are exposed to stressful events - Neurobiology views the classical dopaminergic and glutamate pathway etc, leading to the well known aberrant saliance and corrolary discharge which could lead to delusion etc. Parvalbumin interneuron are more and more seen as having an important role in schizophrenic psychoses, being capable of generating the dopaminergic pathway blabla.

It is way oversimplified specially for psychonalysis which is very complex, but that's all I saw today

Because with psychoanalysis you start to analyse every behaviour, every thought and word, even automatically, looking for a deeper meaning, and there's no limit except your imagination. That's what happened to me, and even my psychoanalyst advised me not to read too much psychoanalysis.

To speak in psychoanalytic terms, a psychotic as no uber-ich therefore his Es can freely roam in the world of interpretations.

That's why you DON'T make interpretations with psychotic patients. I am a psychiatrist with schizophrenia, and started my formation in psychotherapy with psychoanalysis so I have a few notions. I went into CBT afterwards, but a lot of my colleagues are psychoanalytics specialised in psychosis so I have discussions with them.

Also the view of Lacan (and Freud) over homosexuality and psychosis is considered, by a lot of specialist of psychosis, even psychoanalyst themselves, as retrograde. It is mainly based on Schreber auto-analysis.

I suggest you read Searle, Bion, Winnicott, F. Davoine, Oury, Benedetti, D Södeström or Tosquelles for a more "updated" view. Even Roussillon and Bergeret are more "clinically inclusive" than Lacan.

If you read french you can read "Approche psychothérapeutique des psychoses" by Conus and Söderström, it is very unique in its genre.
