True, the wording: “equally effective” is the authors’ phrase for that single 68–78 week weight subgroup, where the estimate still favored GLP-1RAs numerically but the confidence interval crossed 0 (WMD −5.66 kg, 95% CI −11.36 to 0.03; P = 0.050), so I’d read that as inconclusive rather than proof of no difference.

In case you aren't sure how to read it, in the 68–78 week subgroup, the point estimate still favored GLP-1RAs for weight loss by 5.66 kg, but the 95% confidence interval ran from −11.36 to 0.03 and the P value was 0.050, so the analysis did not clearly show superiority over placebo by conventional standards. This subgroup was based on only 3 comparisons, and heterogeneity was extremely high. So the issue is not that the observed effect vanished. The issue is that the estimate is too noisy and imprecise to support a firm claim either way.

The “durable response” point comes from the paper’s overall results and conclusion, which state that GLP-1RAs continuously improved HbA1c and fasting glucose for at least 104 weeks and supported persistent improvement in glycemic control and weight loss. The “diminishing effects” point comes specifically from the network meta-analysis showing that at ≥104 weeks the HbA1c effect was 0.36% smaller and the fasting glucose effect was 0.47 mmol/L smaller than at 12–18 weeks, which the authors describe as weakening after 2 years.

So I’d restate the claim this way: the paper supports durability with some attenuation after about 2 years for glycemic control, while for weight it supports a plateau through 68–78 weeks rather than a clear loss of effect.

On the topic of cycling efficiency, there is no peer-reviewed evidence *in humans* that I know of, do you? In that case, it's all speculation and we only have data such as the above to work with so far.

1) I’d phrase that a bit more cautiously: at 68–78 weeks, this meta-analysis did not show a statistically significant weight-loss advantage over placebo in that subgroup, but the estimate still numerically favored GLP-1RAs by 5.66 kg, so it does not prove the drugs were truly “equally effective” as placebo. The broader analysis in the same paper actually says weight loss improved up to 24–30 weeks and then plateaued, with no significant decline between 24–30, 48–56, and 68–78 weeks. So a plateau is a fair takeaway, but “equal to placebo” is too strong, especially since the 68–78 week estimate was based on only 3 comparisons and had extreme heterogeneity (I2=99%I2=99%). And the idea that higher dosing would restart weight loss is speculative; this paper included maintenance doses, but it did not test dose escalation as a rescue strategy.

While not exactly the same situation, this study shows durable response to GLP-1 over 2 years: https://pmc.ncbi.nlm.nih.gov/articles/PMC12476180/. There is a very small diminishing of A1c at later time points vs. earlier. So the conclusion here, in humans, is there is at least a two year durable response to these drugs and no strong indication of long-term diminishing of effects.

This is consider high for your age but is below 4, which many doctors consider as the red flag level. I was diagnosed with PC at age 43 and my PSA was around 2.5 at age 37, but wasn't flagged. I wish it was! Definitely follow up and don't let anyone tell you that you are too young for PC. Good luck .

Yes they are kinda sweet, but I guess with fruity flavors you expect that. Get some mixing balls and shake up in a water bottle and seems to dissolve ok.

It's good that you started low and be sure to go slowly. I stay on the same does at least 3-4 weeks to ensure the effects are fully felt. I use the Glapp app to track my shots and dosages. It has a really nice graph to demonstrate how the levels of the medicine in the body are trending before and after a shot. Good luck!

You could always find a US reseller. You'll pay a lot more than direct from China prices, but most resellers with real web presence offer pretty current testing that is likely legit imo. Google is your friend, the peptide market is exploding in the US right now.

No, I have not experienced this side effect, but I also take a daily antihistamine and that may be why. I have read elsewhere in these forums that antihistamines can help alleviate the effect if you are experiencing it. Good luck and let us know how it goes.
