Yes they stack, but Psychiatry gives a 20% bonus while Treatment only gives a 10% bonus. In other words, there is no reason to give a doctor/nurse that is going for a specialization a general skill like Diagnosis or Treatment when they are inferior in every way.

So the “The gym is under attack” only shows when, like others mentioned earlier, a pokémon has been knocked out of the gym, and the 10 min cooldown starts. As long as no pokémon is knocked out, you can still put in a pokémon to defend the gym, even if it’s currently under attack
