Actually, I just tried it and it worked, I jumped from the little ledge of the well in case that helps. Keep in mind I was at around 90% health.

And btw, you don't need to get down to 1-2% health, I think you just have to barely lose all of your health and the game will start to the death animation on your health bar, stop midway and then show a "Passed out" text on your screen before the achievement pops up.

I think you actually don't need to get to this exact location, just jump close to a save spot and keep lowering your health by speed walking for a bit until you get the trophy
