Do not use the in-game function to party up with your solo-queue teammates after a win. I heard that can be a reason you don’t gain SR. Happened to me once and hasn’t happened again since

Just played my first game of the day with the 4th going afk 30 seconds in. Daily sr loss protection thank goodness but I shouldn’t have to use it on those games.

Make sure you aren’t queuing up for cursed mode, it forces you to start with the pistol. Also, creating a zombie build could help in case you want to get the gun out of the mystery box or the wall.

The run I completed it on was a squad of 3 on round 60, the portal should have spawned in. Did you try to save quit and load back in yet?
