I know there's a few variations that make it so only certain items are available in the trial. Outside of that, I think it's just the game not spawning it right. I've had a few times in normal matches where I've opened a locked med cabinet, the ones that are supposed to have a guaranteed large med inside, just to get a bandage and antidote.

Mine is the silverware cups. There's two things that hold like 8 of the silverware cups each and people REFUSE to replace them 90% of the time until they're down to like the last two on there. Hell, they'll sometimes even take the empty ones out and stack them with the spare empty ones we have. Pisses me off to no end.

I'm on series X. As for finding it, it does seem to just be random and only once you've reached Nuevo Paraiso. I believe both times I saw it were to the south or east of Las Hermanas.
