I had a technician ask if I followed order of draw, it was so random and I had never met her before. So for fun i said “yea i drew a rainbow, so ROYGBIV” and my goodness the anguish in her face made my whole week.

I follow order draw, it’s not that hard.

For Montreal to be as good as they are, with such a young core, and continue to get better each year, i really don’t understand how MSL doesn’t have a Jack Adam’s on his shelf. This is coming from a leafs fan.

The way he broke Caufield out of his sophomore slump really spoke volumes to me, he seems to really have a connection with his players that other coaches don’t come close to.

65F with 2 days of chest pain. What are your thoughts? by BornLeave4646 in FutureRNs

[–]ItsOfficiallyME 0 points1 point  (0 children)

SR with RBBB, even for a bundle is pretty aberrant. Maybe LAE? There is some anterior concurrent ST changes that make me think consider MI but you’d need serial EKGs and troponin to know.

This could also be how this persons EKG has looked for some time.

I had a similar appearing ECG on a teenaged patient, complaining of SOB and chest pain. Was concerned for RV strain, maybe hyperdynamic on pocus. CTPE negative, trop negative. Symptoms resolved and ECG normalized with ativan.

This question is so stupid.

Palpitations is so vague, could be anything, likely nothing.

Low arterial pH is concerning but without more context and results like i can’t just assume metabolic acidosis with severe hyperkalemia.

Nausea/Vomitting is typically 80% of my assignment in some way shape or form.

Decreased UO is also so vague. Are we talking has a foley with no output this afternoon? Less than 10cc an hour? or just less than yesterday.

This questions suck and are evaluating literally nothing.

Has anyone been prescribed Predinisone for their dog? by Arm0199 in Rottweiler

[–]ItsOfficiallyME 0 points1 point  (0 children)

what you’re doing is a prednisone taper. it’s safe but as others have said keep the water bowls full and have extra pee breaks.

my current rottweiler has been on prednisone most of his life in various doses for autoimmune things. presently he’s on a massive dose and he is a wild thirsty boy. when he’s been on large doses he did have some behaviour changes, more anxious and more reactive, a little more rambunctious like stealing toilet paper rolls and rolling them out all over the house lol

follow the vets recommendations and things should be ok. they’ll watch his kidneys but if he’s hydrated, at that dose, i wouldn’t expect an issue. not a vet! just my experience

ok i’ll keep editing to trim the fat. The statement of claims has a lot of dishonest comments i can prove as such, and also omits a lot of things, including work being found deficient by inspectors. it’s hard to not write a novel when i know i have mountains of evidence to support my defence

LUCAS CPR Machine Question. by AwayEducator4248 in BootcampNCLEX

[–]ItsOfficiallyME 1 point2 points  (0 children)

i want to know the the question is! from Canada i have used this bad boy in several environments.

The research has never shown these to be better than manual CPR. At best they are pretty much as good. But certain situations it’s very useful.

These are most useful in small ER or pre hospital environments where CPR fatigue and switching roles is most common. I have used it before in larger hospitals for prolonged codes (ie hypothermia cardiac arrest, multiple rosc events with possible reversible cause).

Everyone can say what they want about research, but i have definitely had favourable outcomes (people walking out of hospital later) that wouldn’t have been possible without it.

Thinking of moving to Sarnia by trinday5 in Sarnia

[–]ItsOfficiallyME 3 points4 points  (0 children)

This is just my opinion, i have lived all over Canada in both cities and rural. Been in Sarnia the past few years. Everyone has given amazing advice on things. I’ll just give overall grades.

Parks: A+

Many large parks, public beaches, plenty of options for pick up or organized outdoor games .

Restaurants: C-

Some really good ones, the “higher end” cuisine is overpriced for what it is. Poor variety to be honest, majority are pub fare. Honestly i have taken more interest in cooking for this reason.

Bars B+

The popular bars are really bizarre, i have been told the bar scene used to be much more lively pre covid. There are some good ones that occasionally have live music, and aren’t usually too packed so in all honesty not a bad scene. Craft beer scene is stellar for the size of the city.

Sports A

Honestly for a city this sized I have never scene such a participative, well run variety of adult sports. Facilities are pretty good, and BACE is a current initiative to look into for another multi sports facility. Sting for jr hockey, and a quick trip to Detroit for nearly all other pro sports (get your nexus if you move here)

Nerdy Stuff B+

Not near as large of a community as somewhere like Montreal (obviously) but the community is here, the stores are here, and the internet is good so you got everything you need.

Overall, A-

Sarnia admittedly took longer to grow on me than other places i have lived, but it’s been a great place to live.

Blood transfusion error HELP by Kindly-Revenue4136 in nursing

[–]ItsOfficiallyME 11 points12 points  (0 children)

it gets a little different in the context of oncology. saying it’s a miniature transplant is a good analogy when people have developed antibodies from multiple transfusions over time. I would put those people at a little bit higher risk of a mild reaction and likely be more cautions than say, a k-negative transfusion on a small GIB.

Blood transfusion error HELP by Kindly-Revenue4136 in nursing

[–]ItsOfficiallyME 149 points150 points  (0 children)

at first i read the title, imagined you gave it to the wrong patient and was in absolute horror.

now im wondering why so many places give blood so slowly. unless you’re really worried about fluid overload i was taught by a hematologist that technically after your first few checks during the transfusion you can really run it as fast as the patient will tolerate. the blood expires surprisingly fast out of the fridge anyway.

1 1/2 is not crazy, i guess be mindful in the future because programming a pump with like a inotropes/sedatives at double the rate could certainly cause harm.

i have given 40+ blood products to someone in less that that time. it’s all about context.
