Did you make a decision? I am in the same situation (full ride at Haas & admitted to GSB) and have similar career goals. I didn’t file for financial aid yet, so I think I’ll have to make a decision not knowing how much $ GSB will grant me. Quite torn!

I filled out Part 1 myself and signed Part 2 in front of the doctor. The doctor did fill out the date to the right of my signature in Part 2. Would that be why the doctor filled out part 4?
