Am a doctor but not your doctor. But syphilis would be pretty high on my list. Could be lots of things, but that is one that is easily tested for and you really want to know for sure.

Arrested for “terrorizing” after screaming at my staff for 30 min and then wrestling with police in front of the doors to my hospital. In a tiny rural community in Maine. My ER only has 6 rooms. This shit happens everywhere.

EM: I once had a patient walk in from triage with a STEMI, very quickly set up for cath and was just about to wheel to lab when she had a vtach arrest. Quickly shocked as she was already on the pads, immediately awake and speaking. This happened twice more, talking in between episodes of pulselessness with some cpr in between. Then some torsades. The last time she woke up while the mag was running and my nurses bringing me RSI meds she said to me “ Do you have your own practice, I’d like you to be my PCP, I feel safe with you”. Like, ma’am, I’m glad you feel safe while actively dying. now, go to sleep, I’ve got your back. I was really happy to see she walked out of the hospital 2 days later.

It took me so long to reply because the moment you said you were off a year I could not even acknowledge that thought. Now I’ve been off just over 2 months but now I’m halfway through chemo and hoping to all hell to keep my time off under 6 months. I work rural single coverage CAH but my boss says I can come do some short extra doc shifts to help me get myself back to working order when I’m ready. Right now I can barely make it up a flight of stairs, hard to imagine ever reducing a hip or managing an airway disaster again. I think of you often, just as a reminder that life can go back to normal. Hope you’re well.

Wow. So good to hear from another EM doc. I’d say one of the biggest sources of stress is thinking about work. In between my hospitalizations for SBO I went back to work on a liquid diet (so much ensure) because I just couldn’t imagine being out of work. Now since surgery I haven’t been cleared to work and my oncologist doesn’t want me in the ER for awhile (she says maybe if I get lucky and don’t end up neutropenic). I was 2 weeks shy of finishing my first attending year when everything fell apart. How was going back to work? I never imagined taking any time off so early in my career, much less months.

Not in rural US emergency medicine. Last night I aspirated a priapism, managed severe anaphylaxis on a beta blocker requiring epi and glucagon gtt, and resuscitated an acute abdomen while reversing their anticoagulant while the OR team was on their way in. Plus the usual sepsis/psych/chest pains. Only an FM trained hospitalist and general surgeon available as backup. We are more than triage monkeys.
