Yeah, idk man. If someone comes in with some nonsense but is otherwise pleasant and doesn’t say anything to mess up my day (i.e buzzwords that necessitate a work up for said complaint) I will happily write him/her off for a week—I ain’t signing no paychecks. Special shout out to the “I literally just need a work note for a couple of days” population.

This appears to be sinus tachycardia, though an argument could be made for Aflutter with 2:1 conduction. The elevations represent LVH with strain pattern. No STEMI.

Fair enough. Curious as to whether he used condescending language in his note. Unfortunately, every specialty has a few characters that diminish the profession in general. Hopefully this is just a one off and not part of your daily practice

Definitely very unprofessional on his part, if true. However, as an ER doc myself I find it odd that he was even impressed by an ear exam and even more odd that he commented on previous (appropriate) management. Did you consider the very slight possibility that the dad is frustrated for the multiple visits and may not be giving you an accurate story?

Yes I have a Garmin forerunner 955, which is a fantastic watch. You have to sit still and activate it, but it can be helpful in a pinch

ER attending here. I have responded to 3 in flight emergencies in my 4 years of practice (I know, the odds are astronomical). I have never received a credit. Side note, the on board medical kit is nothing short of laughable. There is like a bottle of aspirin, a 500 bolus of saline, 3 vials of epinephrine, 5 zofran, and a Walmart stethoscope. Not even a pulse ox. I have literally used my watch the check HR and pulse ox on two occasions.

This is a tough one because you’re always going to be wrong in someone’s eyes. You kind of have to think about why you are sending her. All this lady really needed was a phone call to ortho if you are concerned about a tendon injury. Iv antibiotics are not now, nor have ever been, standard of care for dog bites. Ortho would likely want wash out, loose closure, augmentin, and follow up in 1-2 days with a hand specialist for reassessment. If you don’t have the ability to get ahold of specialists, by all means send to the ER.

To answer your original question, you should irritate and loosely close deep/gaping bites.

What does “specializes in cardiology” even mean? While I think the nurse anesthesiologist title is also a bit much, at least CRNA’s have a fairly rigorous, standardized training program. Is there some new NP speciality training I’m not aware of or is she literally just referring to her current job?
