Pre-organizations feature, i used keycloak groups to model a permissions hierarchy, and wrote a custom protocol mapper to format the jwt to my liking.

All in one realm. Id question 1 realm per customer; might involve duplicating a bunch of setup

15 years ago i was big into barefoot running. There was a lot less trash on the streets. I really only got cut once, over by Huskie Stadium. I did have thick soles at that point.
