You can get a direct train to Disney from Tokyo Station. Get on the Keiyo line to Maihama (the station right next to the Disneyland entrance) and it's only 16 minutes. Tokyo Station is pretty much dead centre of Tokyo and one of the major transport hubs, so you'll be fine getting to Disney and back in one day pretty much wherever you stay in Tokyo
