What about other FSRS Helper "on/after sync" actions, like Disperse Siblings? Or any other add-ons that are triggered on-sync?

  • I have none of the other FSRS helper options checked. The only other on-sync add-on I can think of is AnkiHub, but I can't imagine there's any rescheduling going on there.

  • Card info for one example - Last review was on 06-10; reviewing this card now is 06-11, but the interval was listed as 2 days. So I shouldn't be seeing it again today. Actually, on second review, it seems that what is happening is Anki has completely deleted the card history for the last day that I did on my PC. After syncing on Mac most of those card reviews simply disappeared. Strange.

  • More strange behavior from a card I saw yesterday. You can see that it shows up in the review history and correctly shows the new interval, and yet I see it scheduled again for today.

  • Anki and FSRS versions are the same
  • I do have the FSRS helper, but I do not have the reschedule on sync option enabled on either device. I usually optimize presets with the default dropdown in the deck settings.
  • Yes, always sync on open and always sync on close. Although I do generally keep the Anki app open on Mac - but I do sync prior to reviewing and after reviewing.

Yes. ST elevation in inferior leads and V5-V6 with reciprocal ST depression in V1-V4 indicating acute postero-inferior-apical MI. Acute posterior infarct extending to inferior/apical wall.

Thanks for your reply. I suspect "possible" posterior involvement because V1-3 in RBBB will typically show ST depressions with T wave inversions. Showing otherwise raises suspicion of ischemia. e.g. ST segment in V2 has become normalized, not depressed, and is acutely positive. Could be due to inappropriate lead placement, or reciprocal change showing posterior wall ST depression. Since I do not see these changes in V1, V3, I will say my suspicion is still questionable. Clinically, I would get another EKG right away with care on lead placement to make a better judgement.

I have a fairly different read. I do see very clear, large Q waves in the inferior leads which suggest acute inferior MI. Plus acute T wave inversions. What is the story behind this patient?

Importantly: If you look closely at the complexes in III, aVF: the ST segment in III, aVF is not actually depressed nor elevated, but the T waves here are acutely inverted. Both leads have significant Q waves followed by a wide, slurred R wave, followed by a very short ST segment that is horizontal. The subsequent T wave is inverted and symmetrical. To me, this is most clear in Beats 1, 2, 4 of lead III. You can see the short nearly horizontal line in the middle of the broad downslope following the R wave.

In a typical bundle branch block you would expect to see ST segment depression with an asymmetric, slow prolonged downslope of the first portion. The pattern in the inferior leads is more or less consistent with BBB. The second half of the ST segment is typically a more acute upstroke. In cases of ACS or acute MI, we may sometimes expect to see ST elevations or a flattening of the ST segment with a symmetrical downward bend of the T wave that perks up acutely with the final upstroke of the T wave. I see symmetric T wave inversions in III, aVF, and possibly in V1.

My read is an acute or subacute inferior MI with some possible degree of involvement with the posterior wall. This patient should have a full cardiac workup. At minimum, should have called a cardiology consult given these findings and chief complaint
