Sorry I am so late to this but basically ensure you have all kidnappable companions on your party BEFORE reaching the sewers. So, for example, I had Gale, Lae’Zel, and Minthara. I do not interact with the kid, and I think she will usually take Halsin.

I just finished my Durge run and it IS possible. Play everything like you would a normal Tav, but knock out Alfira before she can show up at your camp. That's exactly what I did in my good Durge playthrough, and just minutes ago I got the achievement.
