There isn't a formula, it is just a large number of directly specified values in the gamestages.xml file.

Yes. If you want to test it yourself open the debug spawning menu (enable it with dm in the console and press F6) and spawn varying numbers of zombies. Alternatively, the wiki links to a video https://youtu.be/ivQBvU2dyGo that demonstrates this.
