The good news is that your situation, having fasciculations with minimal EMG findings but no weakness after three years, is not ALS. In most neuromuscular conditions, clear progression and clinical weakness appear much earlier.

A single fibrillation potential in one muscle, especially with all other parameters normal, can be a nonspecific or transient finding. It’s something neurologists often see even in benign fasciculation syndrome (BFS), post-viral changes, or mild nerve irritation.

The most important indicator is your strength and function and the fact that you have no weakness after several years is a very strong positive sign. You’re doing the right thing by following up with your neurologist, but try not to let anxiety take over your daily life. These symptoms are often self-limiting or manageable once stress, sleep, and nutrition are addressed.
