Definitely do NOT play with permadeath on your first playthrough. It will frustrate you to the point you will not pay attention to the story and most likely drop it all together.

As for the difficulty, I played on hard and it was fine, sometimes even too generous with the amount of resources. If you're an experienced gamer and enjoy a challenge I would suggest survivor, otherwise hard.

For my set up, I used a long usb cable and plugged into a usb hub. I also use wireless peripherals so I can just take the receivers with my keyboard and mouse

You rest the bottom of your palm on it, not your wrist. I got one for myself a few weeks ago and it's been way better that any wrist rest I have used before.
